もしくは閉塞部に留置することにより、 これら病変部を開存状態に維持する ステントに関する。 より具体的には、 ステ書ント表面から病変部の再狭窄抑制効果 を有する生物学的生理活性物質を徐放、 すなわち少量ずつ長期間にわたって放出 することにより、 これら病変部を長期にわたって開存状態に維持することができ るステントに関する。
背景技術 一つの例として、 虚血性心疾患に適用される血管形成術について説明する。 我が国における食生活の欧米化が、 虚血性心疾患 (狭心症、 心筋梗塞) の患者 数を急激に増加させていることを受け、 それらの心疾患を治療する方法として経 皮的経血管的冠動脈形成術 (P T C A) が施行され、 飛躍的に普及してきて いる。 現在では、 技術的な発展により適用症例も増えており、 P T C Aが始 まった当時の限局性 (病変の長さが短いもの) で一枝病変 (1つの血管にのみ狭 窄がある病変) のものから、 より遠位部で偏心的で石灰化しているようなもの、 そして多枝病変 (2つ以上の血管に狭窄がある病変) へと P T C Aの適用が拡大 されている。 P T C Aとは、 患者の脚または腕の動脈に小さな切開を施して イントロデュ一サーシース (導入器) を留置し、 イントロデューサ一シ一スの内 腔を通じて、 ガイドワイヤを先行させながら、 ガイドカテーテルと呼ばれる長い 中空のチューブを血管内に挿入して冠状動脈の入口に配置した後ガイドワイヤを 抜き取り、 別のガイドワイヤとバルーンカテーテルをガイドカテーテルの内腔に 揷入し、 ガイドワイヤを先行させながらバルーンカテーテルを X線造影下で患者 の冠状動脈中を血管の狭窄部または閉塞部である病変部まで進めて、 バルーンを 病変部内に位置させて、 その位置で医師がバル一ンを所定の圧力で 3 0〜6 0秒 間、 1回或いは複数回膨らませる手技である。 これにより、 病変部の血管内腔は 拡張され開存状態を維持し、 それにより血管内腔を通る血流は増加する。 しかし ながら、 カテーテルによって血管壁が傷つけられたりすると、 血管壁の治癒反応 である血管内膜の増殖が起こり 3 0〜4 0 %程度の割合で再狭窄が報告されてい る。
この再狭窄を予防する方法は、 これまで確立されるに至っていないが、 ステン トゃァテローム切除カテーテル等の器具を用いる方法等が検討され、 ある程度の 成果をあげている。 ここで言うステントとは、 血管あるいは他の管腔が狭窄もし くは閉塞することによって生じる様々な疾患を治療するために、 その狭窄部もし くは閉塞部である病変部を拡張し、 その内腔を開存状態に維持するためにそこに 留置することができる中空管状の医療用具である。 それらの多くは、 金属材料ま たは高分子材料よりなる医療用具であり、 例えば金属材料や高分子材料よりなる 中空管状体の側面に細孔を設けたものや、 金属材料のワイヤや高分子材料の繊維 を編み上げて円筒形に成形したもの等様々な形状のものが提案されている。
.留置の目的は、 P T C A等の手技を施した後に起こる再狭窄の予防、 お よびその低減化を狙ったものであるが、 これまでのところステントのみでは狭窄 を顕著に抑制することができていないのが実情である。
そして近年では、 このステントに抗癌剤等の生物学的生理活性物質を担持させ ることによって、 管腔内のステントを留置した部位でこの生物学的生理活性物質 を局所的に徐放させ、 再狭窄率の低減化を図る試みが盛んに提案されている。 例 えば、 特開平 8— 3 3 7 1 8号公報にはステント本体の表面に治療のための物質 (生物学的生理活性物質) とポリマーの混合物をコーティングしたステントが、 また特開平 9一 9 9 0 5 6号公報にはステント本体の表面に対生物作用材料 層 (生物学的生理活性物質層) を設け、 さらにこの対生物作用材料層の表面 にポリマー製の多孔質材料層を設けたステントが、 それぞれ提案されている。 しかしながら、 特開平 8— 3 3 7 1 8号公報で提案されているステントは、 治 療のための物質 (生物学的生理活性物質) がポリマー中に取り込まれている ため、 ポリマーが生物学的生理活性物質に対して化学的に作用することで生物学 的生理活性物質が分解、 劣化してくるという問題、 すなわち生物学的生理活性物 質の安定性という点で問題が生ずる。 一例を挙げると、 ポリ乳酸をポリマ一とし て選択した場合、 ポリ乳酸は生体内で分解される性質を持っため、 単に生物学的 生理活性物質を体内で放出するという点では優れた機能を発揮するが、 一方でこ のポリ乳酸は分解される時に酸を発生するという性質を持っている。 従って、 生 物学的生理活性物質として酸に弱いものを選択した場合、 ポリ乳酸が分解される ことにより、 生物学的生理活性物質が分解、 劣ィヒしてくるという問題が生ずる。 また、 生体内での分解速度が速いポリマ一を選択した場合、 短期間 (留置後、 数 日内) で生物学的生理活性物質が全て放出してしまうため、 血管内腔の再狭窄を 充分抑制することができないという問題が出てくる。 従って、 生物学的生理活性 物質の分解、 劣化を避け、 なおかつ生物学的生理活性物質が長期間 (留置後、 数 週間から数ケ月) にわたつて放出されるようにするために、 特開平 8— 3 3 7 1 8号公報で提案されているタイプのステントは、 ポリマーと生物学的生理活性物 質の組み合わせという点で、 選択範囲が限定されるという問題がある。
一方、 特開平 9— 9 9 0 5 6号公報で提案されたステントは、 対生物作用材料 層 (生物学的生理活性物質層) とポリマー層が別の層に分かれているため、 ポリマーによる生物学的活性物質の分解、 劣化という点では不安はないが、 生物 学的生理活性物質を覆っているポリマ一層に多孔体を使用しているため、 ポリマー層の一端から他端まで通路が形成されており、 生体内に挿入する前の段 階、 すなわち製造された時点から、 既に生物学的生理活性物質層がステント外雰 囲気 (ポリマー層の外側) にさらされていることになる。 従って、 このような構 造のステントは、 病変部に留置する前に、 生物学的生理活性物質が多孔体の通路 を通ってステントの外側に放出してしまう可能性がある。 また、 病変部に留置し た後も、 生物学的生理活性物質が短期間 (留置後、 数日内) で急激に放出される という現象、 すなわち初期バ一ストが起こりやすく、 生物学的生理活性物質を徐 放、 すなわち少量ずつ長期間 (留置後、 数週間から数ケ月) にわたつて放出させ ることが難しいという問題もあった。 発明の開示
従って、 本発明の目的は、 生物学的生理活性物質の分解、 劣化が起こることが なく、 安定した状態でステント本体に担持させることが可能であり、 なおかつ病 変部に留置した後、 生物学的生理活性物質が短期間で急激に放出されることがな く、 生物学的生理活性物質を徐放、 すなわち、 長期間にわたって少しずつ放出す ることができるステントを提供することにある。
すなわち、 このような目的は、 下記 (1 ) 〜 (1 2 ) の本発明により達成され る。

このような径方向の拡縮可能の構造のステント本体の具体例としては、 例えば 特開平 9— 215753号公報、 特開平 7— 529号公報に開示されているよう な弾性線材をコイル状に屈曲させて、 それを複数接続して円筒形状にされた例で 弹性線材同士のすき間が切欠部をなすステント本体;特表平 8— 502428号 公報およぴ特表平 7— 500272号公報に開示されているような、 弹性線材を ジグザグ状に屈曲させてそれを複数接続して円筒形状にされた例で弹性線材同士 のすき間が切欠部をなすステント本体;特表 2000— 501328号公報およ び特開平 11 -221288号公報に開示されているような、 弾性線材をへび状 平坦リボンの形に曲げて、 これをマンドリルにヘリックス状に巻きつけて 円筒形状にされた例で弹性線材同士のすき間が切欠部をなすステント本体; 特表平 10— 503676号公報に開示されているような、 図 1のステント本体 とは切欠部の形状が異なり、 メアンダー (me a nd e r) 模様の形状であ るメッシュ状の構造をしたステント本体;特表平 8— 507243号公報に開示 されているような、 板状部材をコイル状に屈曲させて円筒形状にされた例で隣接 するコイル部分間のすき間が切欠部をなすステン卜本体等が挙げられる。 また、 特公平 4一 68939号公報には、 弹性板状部材をらせん状に成形して円筒形状 にされた例で隣接するらせん部分のすき間が切欠部をなすステント本体、 弾性線 材を編組して円筒形状にされた例で弾性線材同士のすき間が切欠部をなす ステント本体を含む複数の異なる構造を有する円筒形状のステント本体等が例示 される。 この他、 ステント本体は、 板パネコイル状、 多重螺旋状、 異型管状等で あってもよい。 また、 特公平 4—6 8 9 3 9号公報の図 2 ( a ) , ( b ) には弹 性板状部材を渦巻き状に曲げて円筒形状にしたステント本体が記載されてい るが、 このように円筒体の側面に切欠部を有しないが、 円筒体の径方向に拡縮変 形可能に構成された円筒形状のステント本体も本発明のステント本体として使用 することができる。 これら上記の全ての文献および特許出願は、 引用することで 本明細書の一部をなす。
留置後のステント本体の拡張手段は、 特に限定されず、 自己拡張型、 すなわち 細く小さく折り畳んだステント本体を保持している力を除くことで、 自らの復元 力で半径方向に拡張するタイプのものであってもよく、 バルーン拡張型、 すなわ ちステント本体を内側からバルーンを拡張して外力によって半径方向に拡張する タイプのものであってもよ.い。
ステント本体の材料としては、 高分子材料、 金属材料、 炭素繊維、 セラミック ス等が挙げられ、 ある程度の剛性と弾性を有するものであれば特に制限は ないが、 生体適合性を有する材料であることが好ましい。
具体的には、 高分子材料としては、 例えばポリエチレン、 ポリプロピレン 等のポリオレフイン、 ポリエチレンテレフタレ一ト等のポリエステル、 セル ロースアセテート、 セルロースナイトレ一ト等のセルロース系ポリマ一、 ポリテ卜ラフルォロエチレン、 テトラフルォロエチレン一エチレン共重合体等の 含フッ素ポリマ一等が挙げられる。 金属材料としては、 例えばステンレス、 タンタル、 チタン、 ニッケルチタン合金、 タンタルチタン合金、 ニッケルアルミ ニゥム合金、 インコネル、 金、 プラチナ、 イリジウム、 タングステン、 コバレト 系合金等が挙げられる。 ステンレス鋼の中では、 最も耐食性が良好である s us
3 1 6 Lが好適である。
ステント本体は、 上記例示した材料から、 その適用箇所または拡張手段に応じ て適宣選択した材料により好適に形成することができる。 例えばステント本体を 金属材料で形成した場合、 金属材料は強度に優れているため、 ステントを病変部 に確実に留置することが可能である。 ステント本体を高分子材料で形成した 場合、 高分子材料は柔軟性に優れているため、 ステントの病変部への到達性 (デリバリ一性) という点で優れた効果を発揮する。
また、 ステントが自己拡張型である場合、 元の形状への復元力が必要なことか らチタンニッケル等の超弾性合金等が好ましく、 バルーン拡張型である場合、 拡 張後の形状復帰が起こりにくいことが好ましいことからステンレス等が好ま しい。
また、 ステント本体を炭素繊維で作製した場合、 高強度で、 かつ柔軟性に優れ ており、 しかも生体内での安全性が高いという点で優れた効果を発揮する。 ステント本体の大きさは適用箇所に応じて適宜選択すれば良い。 例えば、 心臓 の冠状動脈に用いる場合は、 通常拡張前における外径は 1 . 0〜3 . 0 mm、 長 さは 5〜 5 0 mmが好ましい。
上記したように、 ステント本体が線状部材で構成される場合、 ステント本体を 多数の切欠部を有するように構成する線状部材の幅方向の長さは、 好ましく は 0 . 0 1〜0 . 5 mmであり、 より好ましくは 0 . 0 5〜0 . 2 mmである。 ステント本体の製造方法は、 特に限定されず、 ステントの構造および材料に応 'じて、 通常使用される製造方法から適宜選択すればよい。
本発明のステントでは、 上記したステント本体を構成する線状部材 2の表面を 覆うように生物学的生理活性物質層 3が形成されている。 ここでステント本 体が、 上記例示した例えば板状部材のような、 線状部材以外の要素で構成される ものである場合、 ステント本体の構成要素、 具体的には板状部材等の表面を覆う ように生物学的生理活性物質層 3を形成する。
生物学的生理活性物質層 3は、 少なくとも 1種類以上の脂溶性の生物学的生理 活性物質を含む。 生物学的生理活性物質層に含まれる生物学的生理活性物質は、 脂溶性であって本発明のステントを生体管腔の病変部に留置した際に再狭窄を抑 制する効果を有するものであれば特に限定されず、 例えば抗癌剤、 免疫抑制剤、 抗生物質、 抗高脂血症薬、 A C E阻害剤、 カルシウム拮抗剤、 抗アレルギー剤、 レチノイド、 抗酸化剤、 抗炎症剤等が挙げられる。
抗癌剤としては、 例えば塩酸イリノテカン、 パクリタキセル、 ドセタキセル水 和物、 メトトレキサ一ト等が好ましい。
免疫抑制剤としては、 例えばシロリムス、 タク口リムス水和物、 ァザチォ プリン、 シクロスポリン、 ミコフエノール酸モフエチル等が好ましい。
抗生物質としては、 例えば、 マイトマイシン C、 塩酸ドキソルビシン、 ァクチ ノマイシン D、 塩酸イダルビシン、 塩酸ピラルビシン等が好ましい。
抗高脂血症剤としては、 より具体的には H M G— C o A還元酵素阻害剤 ゃプロブコールが好ましい。 HMG— C o A還元酵素阻害剤としては、 例えば、 セリバスタチン、 シンパスタチン、 ロバスタチン等が好ましい。
A C E阻害剤としては、 例えば、 トランドラプリル、 シラザプリル、 塩酸テモ 力プリル、 塩酸デラプリル、 マレイン酸ェナラプリル等が好ましい。
カルシウム拮抗剤としては、 例えば二ルパジピン、 塩酸べニジピン、 二ソルジ ピン等が好ましい。
抗アレルギー剤としては、 例えばトラニラストが好ましい。
レチノイドとしては、 例えばオールトランスレチノイン酸が好ましい。
钪酸化剤としては、 例えばェピガロカテキンガレ一トが好ましい。
抗炎症剤としては、 例えば、 デキサメタゾン、 プレドニゾロン等のステロイド やアスピリンが好ましい。
生物学的生理活性物質は、 好ましくは分子量 6 0 0以下の低分子量の生物学的 生理活性物質である。 生物学的生理活性物質が分子量 6 0 0以下の低分子量物質 であると、 ポリマ一層に微粒子が分散されていない場合に、 生物学的生理活性物 質がポリマ一層を通過する速度がある程度高く、 ポリマ一層に微粒子を分散する ことで、 徐放性コーティングから外部への生物学的生理活性物質の放出速度を制 御する上で好ましい。
上記例示した生物学的生理活性物質のうち、 セリバス夕チン、 シンパスタチン および口パスタチンが分子量 6 0 0以下の生物学的生理活性物質の具体例として 挙げられる。
生物学的生理活性物質層は、 上記例示した生物学的生理活性物質のうち、 一種 類のみを含んでもよく、 または二種類以上の異なる生物学的生理活性物質を含ん でもよい。 二種類以上の生物学的生理活性物質を含む場合、 その組み合わせは上 記例示した生物学的生理活性物質から必要に応じて適宜選択すればよい。
ステント本体を構成する線状部材 2の表面に生物学的生理活性物質層 3を形成 する方法は、 ステント本体を構成する線状部材 2の表面に均一に生物学的生理活 性物質層 3を形成することができれば特に限定されず、 例えば、 生物学的生理活 性物質を加熱融解させてステント本体を構成する線状部材 2の表面に塗布 するか、 若しくは加熱融解させた生物学的生理活性物質中にステント本体を浸漬 し、 その後生物学的生理活性物質を冷却し、 固化されることで生物学的生理活性 物質層 3を形成することができる。 また、 生物学的生理活性物質を適当な溶媒に 溶解させて溶液を作製し、 この溶液中にステント本体 2を浸漬し、 その後引き上 げて、 溶媒を蒸散もしくは他の方法で除去することでステント本体を構成する線 状部材 2を被覆する方法、 あるいはこのような溶液をスプレーを用いてステント 本体を構成する線状部材 2に噴霧するか、 または他の手段でステント本体を構成 する線状部材 2の表面に塗布し、 溶媒を蒸散もしくは他の方法で除去することで ステント本体を構成する線状部材を被覆する方法等が挙げられる。
なお、 ステント本体を構成する線状部材 2に対する生物学的生理活性物質の付 着力が不足しており、 生物学的生理活性物質のみではステント本体を構成する線 状部材 2の表面に層を形成することができないと考えられる場合、 粘着性を付与 する追加成分を溶液に加えることが好ましい。 具体的には例えば、 追加成分とし て、 分子量 1 0 0 0以下の低分子量の高級脂肪酸、 例えば魚油、 植物油、 脂溶性 ビタミン、 例えばビタミン A、 ビタミン E等を溶液に加えることが好ましい。 更 に、 生物学的生理活性物質と混合する追加成分の融点が生物学的生理活性物質の 機能を損わない程度に低い場合には、 ステント本体を構成する線状部材 2に被覆 して乾燥させた後で、 上記混合物質の融点以上に加熱することで熱融着させ、 よ り強固に生物学的生理活性物質をステント本体を構成する線状部材 2へ固定する ことが可能となる。
なお、 生物学的生理活性物質を溶解させるのに好適な溶媒が存在し、 かつ、 生 物学的生理活性物質のみでステント本体を構成する線状部材 2の表面に層を形成 することができるのであれば、 生物学的生理活性物質のみを溶媒に溶解させた溶 液に、 ステント本体を浸漬して乾燥する方法、 あるいは前記溶液をスプレーを用 いてステント本体を構成する線状部材 2に噴霧して乾燥する方法が最も簡易であ り、 最も好ましく適用される。
生物学的生理活性物質層 3の量は、 ステントの形状および寸法によるが、 病変 部への到達性 (デリバリー性) や血管壁への刺激性などのステント本体の性能を 著しく損なわない範囲で、 なおかつ生物学的生理活性物質の放出による効果が十 分に発揮される範囲で設定される。 生物学的生理活性物質層 3の量は、 好ましく は 1 0 0〜1, 0 0 0 x g / c m2 である。
本発明のステントでは、 上記した生物学的生理活性物質層 3を覆うように ポリマ一層 4が形成されている。 ポリマー層 4は、 生物学的生理活性物質と相溶 性を有するポリマーで構成され、 ポリマー層 4中に粒径 5 m以下の微粒子 5が 分散されている。
ポリマー層 4を構成するポリマーは、 生物学的生理活性物質と相溶性を有する ため、 生物学的生理活性物質層 3とポリマー層 4の界面部分で生物学的生理活性 物質を溶出させる。 溶出した生物学的生理活性物質は、 ポリマ一層 4を通過して 徐放性コーティングの外部へと放出される。 したがって、 ポリマー層 4が生物学 的生理活性物質と相溶性を有するポリマ一のみで構成されていたならば、 溶出し た生物学的生理活性物質はポリマ一層 4を速やかに通過して、 ポリマー層 4の外 部へと生物学的生理活性物質が短期間で急激に放出されてしまう。 本発明の ステントでは、 このようなポリマー層 4中に粒径 5 m以下の微粒子 5を 分散することで、 生物学的生理活性物質の放出速度を制御する。 すなわち、 ポリマ一層 4中に分散された微粒子 5は、 生物学的生理活性物質がポリマー層 4 を通過する際に障害物として作用し、 生物学的生理活性物質の通過速度を 抑制し、 制御する。 これにより、 ポリマー層 4から生物学的生理活性物質が短期 間で急激に放出されることが防止され、 生物学的生理活性物質が徐放、 すなわち 長期間にわたって少しずつ放出される。
ポリマー層 4を構成するポリマーは、 生物学的生理活性物質層 3を形成する生 物学的生理活性物質と相溶性があれば特に限定されない。 したがって、 ポリマー 層 4を構成するポリマーは、 生物学的生理活性物質層 3を形成する生物学的生理 活性物質に応じて個別に適宜選択される。 ただし、 生物学的生理活性物質は脂溶 性であるので、 ポリマ一層 4を構成するポリマーは一般に脂溶性である。
なお、 生体の管腔内に留置するステントに使用することを考えると、 ポリマー 層 4を構成するポリマーは、 生体安定性が高くて組織刺激性の低いものであるこ とが好ましく、 このようなポリマーとしては、 具体的には例えば、 シリコーンェ ラストマーに代表されるシリコーン系ポリマ一、 セルロース系ポリマ一、 ポリウ レタン、 ポリエステル、 ビニル系ポリマ一、 アクリル系ポリマー、 熱可塑性エラ ストマ一等が挙げられる。 中でも好ましく用いられるのはシリコーンエラ ス卜マ一である。
シリコーンエラストマ一は、 主としてジアルキルポリシロキサンからなるエラ ストマー組成物を広く含み、 直鎖のもの以外に分枝を有してもよく、 ビエル基を 含有するものやアルキル基の一部が水素原子に置換されたものや、 ァミノ変性若 しくはハロゲン変性されたものであってもよい。 ただし、 ジアルキルポリシロキ サンを構成するアルキル基の 90%以上はメチル基である。 また、 硬化前のシリ コーンエラストマ一の粘度は、 500 c p s以上であり、 硬化後のシリコーンェ ラストマーの硬さは、 20〜80 Du r ome t e r Ha r dne s s, s ho r e Aである。 なお、 ポリマ一層 4を形成する際の作業性から、 二液型の シリコーンエラストマ一が好ましい。 上記した条件を満足するシリコーンエラス トマ一は、 市販品を用いてもよく、 例えば、 DOW CORNING社の商品名 S I LAT I C、 Nu s i 1社の商品名 S I L I CONE E 1 a s tme r ( MED— 4211) が好適なシリコーンエラストマ一として例示することができ る。
上記のポリマーは、 本発明の目的を阻害しない範囲で可塑剤、 フイラ一等の成 分を含んでいてもよい。
生物学的生理活性物質層 3の表面をポリマ一層 4で覆う方法、 およびポリマ一 層 4中に微粒子 5を分散させる方法は、 生物学的生理活性物質層 3をポリマ一層 4で完全に被覆することができ、 かつポリマ一層 4中に微粒子 5を均一に、 また は後述するように偏在させて分散することができれば特に限定されない。 例えば、 ポリマーと微粒子 5を溶媒に溶解させて溶液を作製し、 予め生物学的生 理活性物質層 3を設けたステント本体をこの溶液に浸漬して乾燥する方法、 ある いは予め生物学的生理活性物質層 3を設けたステント本体を構成する線状部材 2 に、 スプレーを用いて前記溶液を噴霧して乾燥する方法等が挙げられる。
ポリマー層 4の厚さは、 生物学的生理活性物質層 3と同様、 病変部への デリバリ一性や血管壁への刺激性などステント本体 2の性能を著しく損なわない 範囲で設定する。 ポリマー層 4の厚さは、 好ましくは l〜7 5 i m、 更に好まし くは1 0〜5 0 1!1、 最も好ましくは 2 0〜3 0 mの範囲である。 ポリマー層 4の厚さが 1· m未満の場合、 層中に分散される微粒子 5の粒径がポリマー層 4 の厚さよりも大きくなり、 生物学的生理活性物質層 3を完全に覆う被覆膜として の機能を果たせないおそれがある。 一方、 ポリマー層 4の厚さが 7 5 m超であ ると、 ステント 1自体の外径が大きくなり過ぎ、 ステントの到達性 (デリバリ一 性) 、 特に病変部への到達性 (デリバリ一性) に支障をきたすおそれがある。 本発明のステントにおいて、 ポリマー層 4中に分散される微粒子 5の好ましい —つの態様は、 水溶性物質からなる微粒子 5であり、 好ましくは分子量 1 0 0 0 以下の低分子量の水溶性の塩である。 ここで言う低分子の水溶性塩とは、 酸と塩 基の中和から導かれたイオン化合物をいう。 水溶性物質からなる微粒子 5 は、 ポリマー層 4内に水蒸気または水が侵入した際にこれを吸収して、 溶解して ポリマ一層 4内に島状に点在する水の粒を形成する。 ここでポリマー層 4に侵入 する水または水蒸気は、 ステント 1を病変部に留置した際に、 生体内の管腔に存 在するものであり、 ここで言う水蒸気とは、 生体内の管腔に存在する血液等の体 液が気化されたものである。 水溶性物質が水または水蒸気を吸収することで形成 される水の粒は、 脂溶性の生物学的生理活性物質がポリマー層 4を通過する際に 障害物として作用し、 生物学的生理活性物質が徐放性コーティングから外部への 放出速度を抑制し、 制御する。
上記した特徴により、 微粒子 5が水溶性の塩である場合、 ポリマー層 4は水透 過性または水蒸気透過性である。 本発明のステントにおいて、 ポリマー層 4に分散される微粒子 5の別の好まし い態様は、 無機物質または非水溶性かつ非脂溶性の有機物質からなる不活性物質 である。 このような不活性物質は、 分解しにくく、 生体に対して不活性であり、 かつポリマ一層 4中に分散することで、 生物学的生理活性物質がポリマー 層 4を通過する際に障害物として作用し、 生物学的生理活性物質の放出速度 を制御する。 このような不活性物質としては、 具体的には例えば、 シリカ フィラーに代表されるゲイ素化合物、 グラフアイト、 力一ポンプラック、 フラーレン、 カーボンナノチューブ等の炭素同素体、 ポリテトラフルォロ エチレン (P T F E) 等のフッ素樹脂などが挙げられる。 これらの中でも、 外観 とコストの面からシリカフィラ一および P T F Eが好ましい。
これらの不活性物質は、 生体に不活性であるという特性に悪影響することなし に、 適宜表面処理することが可能であり、 ポリマー層 4中での分散性を高めるこ とができる。
微粒子 5の濃度は、 例えば、 微粒子 5が塩化ナトリウム等の低分子量の水溶性 塩である場合はポリマ一層 4を形成するポリマーの質量に対して 0 . 0 0 0 1〜 1質量%であることが好ましい。
一方、 微粒子 5が不活性物質である場合はポリマ一層 4を形成するポリマーの 質量に対して 1 0 ~ 1 0 0質量%であることが好ましい。
微粒子 5の濃度が、 上記の範囲を超えると、 ポリマ一層 4の機械的物性を大き く損う可能性がある。 また、 微粒子 5の濃度が、 上記の範囲以下になると ポリマー層 4内での立体障害としての作用が不十分となり、 生物学的活性 物質層 3のポリマー層 4への溶解拡散速度およびステント外部への放出速度制御 の効果が十分でない可能性があるため好ましくない。
本発明のステントにおいて、 ポリマー層 4での微粒子 5の分散形態は、 特に限 定されない。 したがって、 図 3に示すように、 微粒子 5は、 ポリマ一層 4中に均 一に分散することに限定されない。 ポリマ一層 4中に微粒子 5を偏在させてもよ レ^ 図 4は、 図 3と同様の部分断面図であるが、 ポリマー層 4中に微粒子 5を偏 在させて分散させた態様を示している。 図 4において、 微粒子 5はポリマー層 4 中に均一に分散されておらず、 ポリマー層 4には、 微粒子 5の濃度が低い部分に はさまれるように、 微粒子 5の濃度が高い部分が層状に存在している。 このよう に、 微粒子 5をポリマー層 4中で偏在させれば、 この微粒子 5の濃度が高い部分 を生物学的生理活性物質が通過する際に、 生物学的生理活性物質の通過が阻害さ れる。 しかもポリマー層 4全体での微粒子 5の濃度は必ずしも高くないの で、 ポリマー層 4全体の微粒子 5の濃度が高い場合のように、 ポリマー層 4の機 械的物性は損なわれない。 このように微粒子 5をポリマー層 4中に偏在させた場 合、 ポリマー層 4の機械的物性を損なうことなしに、 ポリマーの質量に対する微 粒子 5の濃度を、 部分的に 1 0 0質量%超にすることが可能である。
ポリマー層 4への添加時の微粒子 5の粒径は、 ポリマー層 4の外面の平滑性を 失うことがないサイズであれば特に限定されないが、 先述したポリマ一層 4の厚 みを考慮すると、 好ましくは 0 . 0 1〜5 m、 更に好ましくは 0 . 0 3〜 3 m、 最も好ましいのは 0 . 0 5〜 1 mの範囲である。
なお、 ポリマー層 4を形成するポリマーと微粒子 5の選択は、 微粒子 5 がポリマ一層 4に分散可能であつて、 微粒子 5がポリマ一層 4中で障害物として 十分作用することができる限り特に限定されないが、 好ましい組み合わせは、 微 粒子 5が水溶解性物質の場合では、 上記したシリコーンエラストマ一中に、 粒径 0. 01〜1 mの塩ィ匕ナトリウムが、 シリコーンエラストマ一の質量に対して 0. 0001〜1質量%分散されており、 微粒子 5が不活性物質の場合では、 上 記したシリコーンエラストマ一中に、 粒径 0. 01〜1 mのシリカフイラ一若 しくは PTFEが、 シリコーンエラストマ一の質量に対して 10〜100質量% で分散されている。
以下、 本発明を実施例により更に具体的に説明するが、 本発明はこれらに限定 されるものではない。
( 1 ) 生物学的生理活性物質のポリマー膜透過試験
(実施例 1)
A液と B液とを等量になるようにへキサン中に分散させた 2液性シリコーンェ ラストマー (Q 7— 4840 S i 1 a t i c ダウ ·コーニング製) 溶液 (シ リコーンエラストマ一濃度 1 Owt %) に、 シリコーンエラストマ一のポリマー の質量および PTFE微粒子の質量の合計質量に対する濃度が 3 Owt %になる ように PTFE微粒子 (粒径 1 m以下) を添加し、 その溶液をシャーレ上 にキャストして膜厚 30 mのポリマ一膜を作製した。
得られた膜を図 5に示す装置の 2室 (7, 8) の中間部 6にセットした。 そして、 装置の一方の室 7を界面活性剤 ( twe e n 20、 日光ケミカル社 製) を 2wt %添加したシンバス夕チン (S VS) の水溶液 (S VS 50 M g/ ml c on e. で調整し、 膜厚 0. 45 mの ADVANTEC製フィルタで ろ過した溶液) で充填し、 別の一方の室 8を界面活性剤 (twe e n 20) を 2wt%添加した逆浸透 (RO) 水で充填した。 この状態で装置を 37 °Cの恒 温槽中に放置し、 SVSのポリマー膜透過速度 (cmZs e c) を測定した。 な お、 透過速度は、 所定時間毎に室 8中の溶液 lm 1を採取して、 分光光度計
(UV- 2400 P C Sh imadz u製) を用いて波長 238 nm部分の吸 光度を測定し、 予め作成しておいた検量線を用いて SVSの膜透過量を算出し、 これを単位時間および膜の単位面積当たりに換算して求めた。 得られた結果 を表 1に示した。
(実施例 2)
A液と B液とを等量になるようにへキサン中に分散させた 2液性シリコーンェ ラストマ一 (Q 7 _ 4840 S i 1 a t i c ダウ ·コ一ニング製) 溶液 (シ リコーンエラス卜マー濃度 1 Owt %) に、 シリコーンエラストマ一のポリマー の質量および塩化ナトリゥムの質量の合計質量に対する濃度が 0. 25 w t %に なるように塩化ナトリウム (粒径 1 m以下) を添加し、 その溶液をシャーレ上 にキャストして膜厚 30 mのポリマー膜を作製した。 得られたポリマー膜につ いて、 実施例 1と同様の方法で SVSのポリマー膜透過速度を測定した。 得られ た結果を表 1に示した。
(比較例 1 )
A液と B液とを等量になるようにへキサン中に分散させた 2液性シリコーンェ ラストマ一 (Q 7— 4840 S i 1 a t i c ダウ 'コーニング製) 溶液 (シ リコーンエラストマ一濃度 10wt%) を、 PTFE微粒子または塩化ナトリウ ムを添加せずに、 シャーレ上にキャストして膜厚 30 mのポリマー膜を作製し た。 得られたポリマー膜について、 実施例 1と同様の方法で SVSのポリマー膜 透過速度を測定した。 得られた結果を表 1に示した。 表 1 SVSのポリマ一膜透過速度 (cm/s e c) 実施例 1 2. 713 E- 05

比較例 1 3. 697 E- 05 表 1より、 P T F E微粒子または塩化ナトリゥムを添加した実施例のポリマー 膜は、 SVSの膜透過速度が遅くなつていることが確認された。
(2) 生物学的生理活性物質の徐放率の測定
(実施例 3)
図 1に示す外径 1. 8 mm、 長さ 15 mmの円筒形状で、 略菱形の切り欠き部 を有するステント本体 (材質: SUS 316 L) を構成する線状部材 (幅: 0. lmm) の表面に、 S VSをへキサン中に分散させた溶液 (SVS濃度 5 w t %) を、 ハンドスプレー (HP— C IWATA製) により噴霧し、 約 200 gの S V Sがステント本体を構成する線状部材の表面に塗布されてい ることを確認した。 溶媒であるへキサンを完全に揮発させて、 ステント本体を構 成する線状部材の表面に生物学的生理活性物質層 (SVS層) を形成した。 その後、 A液 1 gと B液 1 gとをへキサン 98 g中に分散させた 2液性 シリコーンエラストマ一 (Q 7— 4840 S i l a t i c ダウ ·コ一二ング 製) 溶液 (シリコーンエラストマ一濃度 2 wt %) に、 シリコーンエラストマ一 のポリマーと PTFE微粒子の質量の合計質量に対する濃度が 3 Owt %になる ように PTFE微粒子 (粒径 1 m以下) を添加し、 その溶液をノ により噴霧した。 ポリマー層が生物学的生理活性物質層を完全に覆つたことを確 認した後、 80°Gのオーブン中で 1時間加熱硬化させ、 ポリマー層を形成した。 加熱硬化後のポリマ一層の厚さは平均 30 mであった。 そして、 この加熱硬化 したステントについて SVS放出量の測定を行った。
測定は、 界面活性剤 ( twe e n 20) を 2 w t %添加した R〇水中に上 記ステントを浸漬した後、 その溶液を 37 °Cの恒温槽中に放置し、 所定時間毎に 上記溶液 lmlを採取して、 SVS放出量を測定した。 SVS放出量の測定は、 実施例 1と同様な方法で実施した。 結果を SVS徐放率として表 2に示した。 な お、 表 2の数値は、 ステント本体を構成する線状部材に塗布した S V Sの量に対 する SVSの放出量の割合 (%) を示している。
(実施例 4)
実施例 3と同一のステント本体の表面に SVSをへキサンに分散させた溶液 ( SVS濃度 5wt%) を、 ハンドスプレーにより噴霧し、 約 200 zgの SVS がステント本体を構成する線状部材の表面に塗布されていることを確認した。 溶 媒であるへキサンを完全に揮発させて SVS層を形成した。 その後、 A液 l gと B液 1 gとをへキサン 98 g中に分散させた 2液性シリコーンエラストマ一 (Q7-4840 S i l a s t i c ダウ■コーニング製) 溶液 (シリコーン 濃度 2wt%) 中に、 シリコ一ンエラストマ一のポリマーと塩化ナトリウムの質 量の合計質量に対する濃度が 0. 25wt%になるように塩化ナトリウム (粒径 以下) を添加し、 その溶液をハンドスプレーにより噴霧した。 ポリマー層 が生物学的生理活性物質層を完全に覆ったことを確認した後、 80°Cのオーブン 中で 1時間加熱硬化させ、 ポリマー層を形成した。 加熱硬化後のポリマー層の厚 さは平均 30 mであった。 そして、 得られたステントについて SVS放出量の 測定を行った。 なお、 測定は実施例 3と同様の方法で行った。 結果を SVS徐放 率として表 2に示した。 なお、 表 2の数値は、 ステント本体を構成する線状部材 に塗布した SVSの量に対する SVSの放出量の割合 (%) を示している。 (比較例 2)
実施例 3と同一のステント本体の表面に、 S V Sをへキサンに分散させた溶液 (3¥3濃度5 1: %) を、 ハンドスプレーにより噴霧し、 約 200 x gの S VSがステント本体を構成する線状部材の表面に塗布されていることを確認し た。 溶媒であるへキサンを完全に揮発させて SVS層を形成した。 その後、 A液 1 gと B液 1 gとをへキサン 98 g中に分散させた 2液性シリコーン エラストマ一 (Q 7— 4840 S i l a s t i c ダウ ·コ一二ング製) 溶液 (シリコーン濃度 2wt%) を、 PTFE微粒子または塩化ナトリウムを添加せ ずにハンドスプレーにより S V S層に噴霧した。 ポリマ一層が S V S層を完全に 覆ったことを確認した後、 80°Cのオーブン中で 1時間加熱硬化させ、 ポリマー 層を形成した。 加熱硬化後のポリマー層の厚さは平均 30 mであった。 そして、 この加熱硬化したステントについて SVS放出量の測定を行った。 なお、 測定は実施例 3と同様の方法で行った。 結果を表 2に示した。 なお、 表 2の数値は、 ステント本体を構成する線状部材に塗布した S V Sの量に対する SVSの放出量の割合 (%) を示している。 表 2 S VS徐放率 (%)

表 2より、 ポリマ一層に P T F E微粒子または塩化ナトリゥムを添加した実施 例のステントは、 SVSがゆつくり徐放されていることが確認確認された。 産業上の利用可能性
以上述べたように本発明は、 生体内の管腔に留置するためのステントで あって、 その両末端部が開口し、 該 2つの末端開口部の間に長手方向に延在する 円筒状のステント本体と、 該 本体の表面に形成される生物学的生理活性 物質の徐放性コーティングと、 を含み、 前記徐放性コ一ティングは、 前記 ステント本体の表面を覆うように形成された生物学的生理活性物質層と、 前記生 物学的生理活性物質層上に、 該生物学的生理活性物質層を覆うように形成された ポリマ一層よりなり、 前記生物学的生理活性物質層は、 少なくとも一種類の脂溶 性の生物学的生理活性物質を含有し、 前記ポリマー層は、 前記生物学的生理活性 物質と相溶性を有するポリマーと、 該ポリマー中に分散された粒径 5 ix m以下の 微粒子と、 を含むことを特徴とするため、 生物学的生理活性物質の分解、 劣化が 起こることがなく、 安定した状態でステント本体に担持させることが可能 である。 そして、 病変部に留置した後、 生物学的生理活性物質が短期間で急激に 放出されることがなく、 長期間にわたって少しずつ放出される。
本発明において、 ステント本体が金属材料で形成されている場合、 金属材料は 強度に優れているため、 ステントを病変部に確実に留置することが可能である。 本発明において、 ステント本体が高分子材料で形成されている場合、 高分子材 料は柔軟性に優れているため、 ステントの病変部への到達性 (デリバリ一性) と いう点で優れた効果を発揮する。
本発明において、 生物学的生理活性物質層が、 生物学的生理活性物質のみによ り形成されている場合、 簡易な方法で生物学的生理活性物質層を設けることが可 能である。
本発明において、 生物学的生理活性物質は、 抗癌剤、 免疫抑制剤、 抗生 物質、 抗高脂血症薬、 A C E阻害剤、 カルシウム拮抗剤、 抗アレルギー剤、 レチノイド、 抗酸化剤、 抗炎症剤のいずれかであることにより、 病変部に ステントを留置した際に、 病変部の再狭窄を抑制することが可能である。
本発明において、 ポリマー層を形成するポリマ一が、 シリコーン系ポリマーで ある場合、 生体内に対して特に優れた安全性を示す。
本発明において、 微粒子が分子量 1 0 0 0以下の低分子量の水溶性塩であり、 かつ生体内に存在する塩であることを特徴とする場合は、 塩の生体に対する刺激 性が低いため、 安全性が高い。 さらに、 低分子量の水溶性塩が、 塩化ナトリウムである場合、 塩化ナトリウム の生体内存在率が高いため、 さらに安全性が高まる。
また、 微粒子がシリカフイラ一または P T F Eのような不活性物質である場合、 生体に不活性であるという特性に加えて、 様々な表面処理が行えるため、 ポリマー層での分散性および相容性が高まる。

Claims

請 求 の 範 囲
1 . 生体内の管腔に留置するためのステントであって、
その両末端部が開口し、 該 2つの末端開口部の間に長手方向に延在する円筒状 のステント本体と、
該ステント本体の表面に形成される生物学的生理活性物質の徐放性コ一ティン グと、 を含み、
前記徐放性コーティングは、 前記ステント本体の表面を覆うように形成された 生物学的生理活性物質層と、
前記生物学的生理活性物質層上に、 該生物学的生理活性物質層を覆うように形 成されたポリマー層よりなり、
前記生物学的生理活性物質層は、 少なくとも一種類の脂溶性の生物学的生理活 性物質を含有し、
前記ポリマー層は、 前記生物学的生理活性物質と相溶性を有するポリマーと、 該ポリマー中に分散された粒径 5 m以下の微粒子と、 を含むことを特徴とする ステント。
2 . 前記ステント本体が、 金属材料で形成されていることを特徴とする請求項 1 に記載のステント。
3 . 前記ステント本体が、 高分子材料で形成されていることを特徴とする 請求項 1に記載のステント。
4. 前記生物学的生理活性物質層が、 生物学的生理活性物質のみで形成されてい ることを特徴とする請求項 1ないし 3のいずれかに記載のステント。
5 . 前記生物学的生理活性物質が、 抗癌剤、 免疫抑制剤、 抗生物質、 抗リウマチ 剤、 抗高脂血症薬、 A C E阻害剤、 カルシウム拮抗剤、 抗アレルギー剤、 レチノイド、 抗酸化剤、 抗炎症剤のいずれかであることを特徴とする請求項 1な いし 4のいずれかに記載のステント。
6 . 前記生物学的生理活性物質は、 分子量が 6 0 0以下であることを特徴とする 請求項 1ないし 5のいずれかに記載のステント。
7 . 前記ポリマー層を形成するポリマーが、 シリコーン系ポリマーであることを 特徴とする請求項 1ないし 6のいずれかに記載のステント。
8 . 前記微粒子が、 分子量 1 0 0 0以下の低分子量の水溶性の塩であることを特 徴とする請求項 1ないし 7のいずれかに記載のステント。
9 . 前記低分子量の塩が、 塩ィヒナトリウムであることを特徴とする請求項 8に記 載のステント。
1 0 . 前記微粒子は、 水不溶性の物質であることを特徴とする請求項 1ないし 7 のいずれかに記載のステント。
1 1 . 前記水不溶性の物質は、 シリカフィラーであることを特徴とする請求 項 1 0に記載のステント。
1 2 . 前記水不溶性の物質は、 ポリテトラフルォロエチレンであることを特徴と する請求項 1 0に記載のステント。
