The present invention relates to a device for connecting hollow organs or implant parts with the features of the preamble of patent claim 1.
In medical operations on the human body, the situation often arises that separated hollow organs, such as arteries, veins, lungs, esophagus or intestinal areas, have to be connected to one another. This can be done either by directly sewing the corresponding hollow organ sections or using appropriate molded parts, which are each fastened to the ends of the hollow organs and which are then brought into engagement with one another in a non-positive or positive manner. The last-mentioned connection is used in particular when vascular prostheses, heart valves, cannulae of blood pumps or other hose-like connections are used. For example, today artificial heart valves, which are composed of the closing body, which consists of one or more parts and forms the valve mechanism of the valve, the valve housing and the seam ring as main elements, are fastened in that the seam ring engages in a groove in the valve housing. The flap unit is fastened in the tissue of the natural heart by the seam ring, such a connection device consisting of two molded parts (seam ring and groove) no longer being separable without destroying the seam ring.
However, the limited lifespan of artificial heart valves often requires replacement of the artificial heart valves. In this renewed operation, the suture ring, including the partially adhering tissue, is cut out of the valve annulus so that the new valve can then be sewn back into the freely prepared annulus. Such a new operation naturally represents a further, additional risk.
In addition, a releasable connecting device is also known in the medical field, which is also commonly referred to as a snap ring connection. In the case of such a ring snap connection, a so-called bead angle is positively locked into a partner molded part when the individual parts are joined, with at least one of the two molded parts having to be temporarily mechanically deformed during this locking process. For this purpose, the action of corresponding deformation forces is necessary, but these deformation forces cannot or cannot be applied to the respective molded parts, depending on the respective operating site, or without a corresponding risk for the patient. This is especially true for all open heart surgery. The present invention has for its object to provide a device of the type specified, which enables a particularly quick., Releasable connection without applying high deformation forces.
This object is achieved according to the invention by a device with the characterizing features of patent claim 1.
According to the invention, a device for connecting hollow organs, in particular for connecting arterial, vein, lung, esophagus or intestinal sections, and for connecting an implant part f, preferably a heart valve, to the surrounding tissue is proposed, the device providing two to the End of the corresponding hollow organs or the tissue and the implant part can be attached ^ molded parts. Here, these molded parts are designed in a ring-like or ring-like manner and can be brought into releasable engagement with one another in a force-fitting and / or positive manner. At least one of these molded parts comprises a memory material which is arranged on the molded part or the molded parts in such a way that the engagement of the molded parts can be produced by a first change in temperature and this engagement can be released again by a second change in temperature.
Due to the fact that the memory material only requires a corresponding temperature change in order to engage the corresponding molded parts in a force-fitting and / or form-fitting manner and, if necessary, also in a fluid-tight manner, the device according to the invention, in contrast to those described above conventional ring snap connections no deformation forces maneuverable to connect the corresponding hollow organ sections with each other. This in turn means that the device according to the invention can also be used during the operation in those areas which are either difficult to access for the surgeon or in those uncontrolled movements which are required when applying the high deformation forces in the conventional snap-in connections are absolutely to be avoided. The device according to the invention can therefore also be used excellently in the cardiovascular area. If, for example, a ring made of such a memory material is used to fix the seam ring on the flap housing, all that is required is the temperature change mentioned above, in order to remove the seam ring on the flap housing previously sewn to the surrounding tissue and thus also the ent to fix speaking tissue on the valve body of an artificial heart valve to be inserted into the heart. However, since such a connection can be released again by a simple second change in temperature, the burdens and risks of replacing one designed in this way are
Herzklappe deutlich verringert. Heart valve significantly reduced. In the case of such a reoperation, there is no renewed injury to the annulus, since the suture ring has already grown into the corresponding tissue during the first operation and does not have to be removed during the reoperation. Furthermore, the device according to the invention permits precise positioning of the molded parts, since the molded parts, even if they are fixed in an unfavorable position relative to one another, can only be released by a change in temperature, so that one molded part can be easily rotated relative to the other molded part . This is very advantageous, in particular, for the heart valve described above, since it enables a retrospective orientation of the valve in the heart. There is also the danger a closing body glue is eliminated by sutures fixing between the seam ring and the closing body, this risk being present in the known heart valves described above. Furthermore, the two molded parts used for the connection, for example the suture ring and the heart valve, can be manufactured with significantly greater tolerances and independently of one another in comparison to the known ring snap connections. Furthermore, such a seam ring made of a memory material gives the relatively flexible flap housing additional stability and thus prevents warping of the flap housing, which can lead to possible malfunctions of the closing body, when sewing in.
A further advantage is the radiological representability of the valve ring provided by the introduction of a metallic memory ring. Damage to the soft sails in heart valves or other easily destructible parts of bioprostheses (implant parts) is also excluded, since the heart valves or bioprostheses are only retrofitted after the suture ring or molded part has been sewn in.
Zuvor wurden die Vorteile der erfindungsgemäßen Verbindungs- einrichtung überwiegend am Beispiel der Implantation vonBefore, the advantages of the connecting device according to the invention were predominantly based on the example of the implantation of
Herzklappen beschrieben. The advantages mentioned there also apply in particular when other hollow organs, such as the hollow organ sections mentioned at the beginning, are connected to one another with the aid of the connecting device according to the invention. The connecting device according to the invention can thus also be used particularly advantageously for connecting vascular prostheses, cannulas of blood pumps or other hose-like connections, wherein - 6 -

If, in addition, the one molded part (ring) is made entirely of a memory material, double engagement with the second molded part can be produced, which ensures particularly high security. In this case, one molded part (ring) is pushed onto the * -> other molded part and is additionally secured via the snap elements arranged over the circumference of the one molded part.
In order to ensure an arrangement of the two molded parts on the hollow organ, these fastening elements can have ducks for the hollow organ sections, these fastening elements preferably made of a textile material, e.g. a polyester yarn material or a polyester fabric, or a plastic film, in particular polytetrafluoroethylene film or derivatives thereof.
- / -Another embodiment of the connecting device according to the invention provides that the molded parts or a molded part does not include any of the fastening elements described above. Here, one molded part consists of the corresponding ring-shaped or ring-shaped memory material, while the other molded part is adapted to this in its design. In order to connect hollow organs to this, the other molded part is arranged within the hollow organ in this way. - / -
die Verbindung der Formteile kraft - und/oder formschlüssig erfolgt.the connection of the molded parts is non-positive and / or positive.
A first embodiment of the connecting device according to the invention provides that one molded part can be pushed onto the other molded part. Here, the one molded part, which can be pushed onto the other molded part, preferably consists of the memory material. Here, the two molded parts are first fixed to the corresponding hollow organs to be connected. Subsequently or beforehand, the one molded part, which consists of the corresponding memory material, is brought to a temperature at which this molded part has a diameter that is larger than the diameter of the other molded part. Now, these two form j ~ parts are pushed into one another without requiring a larger force is required. The corresponding temperature change is then brought about. which leads to the fact that the one molded part, which consists of the memory material, reduces its diameter and shrinks accordingly onto the "other molded part, whereby the engagement and, if necessary, a flush engagement of the two molded parts is achieved the one molded part can be designed as a ring.
Wahlweise kann hierbei der Ring selbstA further embodiment of the device according to the invention provides that the one molded part is designed as a ring, this ring comprising a plurality of snap elements made of the memory material and arranged over the circumference of the ring. The ring itself can be used as an option

Danach erfolgt eine Temperaturänderung, wodurch das ringartige bzw. ringförmige eine Formteil auf das andere Formteil aufschrumpft.30 also consist of a memory material or be made of another material. The other molded part that is connected to it then has corresponding elements that can be brought into engagement with the snap elements. To connect these two molded parts then one that the two sections to be joined above the mold ¬ part overlap. Subsequently, the ring-like or annular form a part, the ration before or during Ope ¬ was brought to a first temperature, slid over the other mold part and the overlapping Hohlorganab ¬ sections. A temperature change then takes place, as a result of which the ring-like or annular one molded part shrinks onto the other molded part.
The embodiments described above always assumed that the one molded part consists of the memory material. Of course, it is possible that other molding made from the to customize memory material or to form both of the mold parts from a memory material, wherein the - ~ - ^ latter case, the material selection is coordinated such that turänderung also by the required temperature ¬ the necessary positive and / or non-positive connection results.
Selbst¬ verständlich ist es jedoch auch möglich, solche MaterialienWith regard to the memory material, it should generally be stated that in the present description, this should be understood to mean those materials which preferably have a repeatable, reversible memory effect. Of course, it is also possible to use such materials

Advantageous further developments of the connecting device according to the invention are given in the subclaims.
Es zeigen:The connecting device according to the invention is explained in more detail below with reference to two embodiments in conjunction with the drawing. Show it:
Figur 1 eine schematische, teilweise perspekti¬ vische Schnittansicht einer ersten Aus¬ führungsform der Verbindungseinrichtung; 5 undFIG. 1 shows a schematic, partially perspective sectional view of a first embodiment of the connecting device; 5 and
CFigure 2 is a schematic, partially sectional view of a second embodiment of the connecting device. C
Here, the partially perspective and sectioned connection device 1, which is used for an artificial heart valve, has a molded part 2, which is designed as a suture ring, as well as another molded part 3 interacting with it, the other molded part 3 being one Forms part of the valve body of the heart valve. The upper part of FIG. 1 shows the connecting device 1 in a position in which the two molded parts 2 and 3 are in engagement with one another, whereby a fluid-tight, detachable connection of the two molded parts 2 and 3 is achieved. This connection can be released by a change in temperature and can be restored by a further change in temperature, which is indicated by the arrow 4. To achieve this, the molded part 2 (seam trin) made of a memory material, the diameter of the molded part 2 changing as a function of an induced temperature change, as is indicated by the area between the arrows 5. Specifically, this means that the molded part 2 has a diameter at 20 ° C, as indicated by the left arrow 5. This diameter is larger than the diameter of the molded part 3, so that the molded part 2 can be placed over the molded part 3, as can be seen in the upper part of FIG. 1. Is raised in this position, the temperature to 37 ° C, so verrin ¬ the diameter of the molded article 2 siege to a value like this is indicated by the right arrow. 5 This causes the molded part 2 to shrink onto the molded part 3, as a result of which a positive connection between the two molded parts 2 and 3 is achieved.
For an exclusively form-fitting connection of the two molded parts 2 and 3, it is only necessary to adapt the diameters of the parts 2 and 3 to one another accordingly and, if necessary, to additionally provide correspondingly shaped sealing surfaces to be brought into contact.
Diese, aus den Dichtungsflächen 7 und 8 sowie demIn order to additionally achieve a positive connection and thus an increase in tightness, the molded part 2 has a shape as can be seen in the enlarged sectional view. Here, the molded part 2, seen in cross section, has a projecting region 6 which is U-shaped, further sealing surfaces 7 and 8 being provided below this region 6. This, from the sealing surfaces 7 and 8 and the
Area 6 existing inner surface of the molded part 2 then comes into fluid-sealing engagement with a groove 9 correspondingly adapted to its shape, so that in addition to the previously described non-positive connection positive connection occurs. To increase the stability, the outer area 10 of the molded part 2 is profiled accordingly, as can be seen in the figure.
Auch hier ist die so hergestellte Verbindung lösbar, wie dies durch denThe second embodiment shown in FIG. 2 of the connecting device, which is denoted overall by 1, likewise has two molded parts 2 and 3, which can be brought into engagement with one another to produce the connection. Here, too, the connection thus established can be released, as can be done by the
With the exception of the configuration of the molded part 2, the embodiment shown in FIG. 2 does not differ in its configuration. This embodiment is also shown using the example of an artificial heart valve, with one part of the valve housing forming the other 1 c molded part 3.
Das als Nahtring ausgebildete Formteil 2 weist eine Vielzahl von über den Umfang verteilten Schnappelementen 11, wie dies am besten der vergrößerten Schnittansicht in der linkenThe shaped part 2 designed as a seam ring has a plurality of snap elements 11 distributed over the circumference, as best shown in the enlarged sectional view in the left
In this case, these snap elements, which bring about a latchable connection, consist of the memory material. These snap elements are arranged on the molded part 2 (seam ring) in such a way that when the temperature rises from 20 ° C. to 37 ° C. they are pivoted by the amount shown between the arrows 5.
Ist zu¬ sätzlich zu dieser überwiegend kraftschlüssigen Verbindung - 12 -This means that in the event of a temperature change in the aforementioned range, the snap elements 11 are pivoted from a position 0, which is indicated by the left arrow 5, into a position, which is indicated by the right arrow 5. As a result, the fluid-tight engagement between the two molded parts 2 and 3 is established. In addition to this predominantly non-positive connection - 12 -
noch eine formsrhlüssige Verbindung erwünschl . r.o besteht die Möglichkeit, an dem Formteil 3 eine in seiner Ausgestal¬ tung an die Schnappelemente angepaßte Nut vorzusehen, wie dies vorstehend im Zusammenhang mit der Figur 1 dargestellt wurde.another positive connection. ro is the possibility of providing on the molding 3 is a ¬ Ausgestal in its processing to the snap-on elements adapted to the groove, as was shown above in connection with FIG. 1
If such a connecting device is used in connection with an artificial heart valve, the seam insert ring forms one molded part 2, while an area of the valve housing represents the other molded part 3. The connections can be positive, non-positive or positive and non-positive. In the case of the non-positive connection, it was found that the textile suture does not have to completely surround the seam insert ring in order to achieve a secure fixation of the heart valve. Depending on the configuration of the molded parts by selecting appropriate cross-sectional configurations, it is possible that the stability of the connection device is increased significantly with little material or volume expenditure. Specifically, in the previously described artificial heart valve, either the seam insert ring can be designed in the form of a thin wire in order to secure this relatively unstable molded part on a relatively stable valve housing as a second molded part. It is also possible to select a rather thin-walled flap housing as the second molded part in order to connect it to a seam ring stabilized by a corresponding design, for example the seam ring 2 shown in FIG. 2.
The embodiments of the connecting device described above using the example of an artificial heart valve are used as follows when implanting an artificial heart valve: First, the seam ring (molded part 2) is sewn with the natural tissue. In this case, the seam ring is brought to a temperature either before the operation or during the operation, in which the seam ring is expanded from the memory material, ie in the specific case of exemplary embodiments 1 and 2 to a temperature of 20 ° C. The flap housing (molded part 3) is then pushed into the widened seam ring and aligned accordingly. The seam ring, including the surrounding area, is then heated to a temperature at which the seam ring is positively and / or non-positively connected to the valve body, that is to say in a specific case to a physiological temperature of 37 ° C. If it is necessary, an implant so implanted To replace artificial heart valve, it is only necessary in a reoperation to cool the memory material with the repeatable memory effect to the above-mentioned temperature, whereby the heart valve used can be replaced by a new heart valve. The connection to the suture ring that has grown into the tissue is then carried out by appropriate heating to the physiological temperature.
