Description
- Hintergrund der Erfindung
- Diese Erfindung bezieht sich insbesondere auf Stents, aber auch allgemein auf jede expandierbare, implantierbare Endoprothese, wie zum Beispiel Implantate, Stent-Implantate und Ähnliches, für die das Dokument WO 98/11846A den nächstliegenden Stand der Technik bildet. Wenn der Begriff "Stent" hierin verwendet wird, ist er in einer allgemeinen Bedeutung zu verstehen, die alle solchen expandierbaren Prothesen einschließt, solange es nicht anders angezeigt wird.
- Die Verwendung von Endoprothesen, zum Beispiel von Stents, Stent-Implantaten und Implantaten ist allgemein bekannt zur Aufrechterhaltung der Durchgängigkeit von Körpergefäßen, einschließlich der Blutgefäße und Gallenwege. Üblicherweise wird eine Endoprothese in ein Gefäß implantiert, das verstopft ist, das zu einem Aneurysma neigt, das momentan eine Läsion besitzt oder anderweitig beschädigt ist. Oftmals wird das Gefäß während der Implantation der Endoprothese unter einer Verletzung leiden. Die Verletzung kann als ein Ergebnis der Gefäßerweiterung vor der Implantation der Endoprothese, der Anwesenheit eines Fremdkörpers (der Endoprothese) im Körpergefäß oder als ein Ergebnis anderer Ursachen auftreten. Ungeachtet des Ursprungs der Verletzung, kann sich das Gefäß als ein Ergebnis der Implantation einer Endoprothese in einem geschwächten und entzündeten Zustand befinden. Obwohl es wünschenswert ist, im Gefäß einen so großen Durchmesser wie möglich beizubehalten, um die Möglichkeit der Restenose zu minimieren, begrenzt die Schwächung des Gefäßes als Ergebnis der Verletzung den Umfang, bis zu dem sich das Gefäß erweitern kann.
- Die Endoprothesen, die zurzeit verfügbar sind, sind üblicherweise mittels eines Ballons expandierbare, selbst-expandierbare oder ballonunterstützt selbstexpandierbare Vorrichtungen. Mittels eines Ballons expandierbare Stents erreichen ihren maximalen Durchmesser nach der Erweiterung durch einen Ballon. Sobald sie in das Gefäß eingesetzt worden sind, sind sie zu einer weiteren Expansion solange nicht imstande, bis ein Ballon wieder in den Stent eingeführt und expandiert wird. Andererseits üben selbst-expandierende und ballonunterstützt expandierbare Stents ständig einen nach außen führenden Druck aus, während sie versuchen, ihren maximal möglichen Durchmesser zu erreichen. Selbst nachdem er implantiert wurde, fährt der Stent daher fort, zu versuchen, sich weiter zu öffnen, indem er einen Druck auf die Gefäßwände ausübt.
- Es wäre wünschenswert, eine Endoprothese bereitzustellen, die im Anschluss an das Einsetzen einige der Merkmale der mittels Ballon expandierbaren Stents besitzt, und die dazu einige der Eigenschaften der selbst-expandierenden Stents nach einer vorherbestimmten Zeitspanne oder nach der Ausübung von Kraft bzw. Druck in einem vorherbestimmten Umfang besitzt. Insbesondere ist es wünschenswert, eine Endoprothese bereitzustellen, welche die mit einem selbst-expandierenden Stent verbundenen nach außen gerichteten Kräfte nicht auf die Gefäßwände überträgt, während das Gefäß sich von der Verletzung durch das Einsetzungsverfahren erholt und den nach außen gerichteten Druck eines selbst-expandierenden Stents noch liefert, wenn sich das Gefäß von der Verletzung ausreichend erholt hat.
- Es ist ebenfalls wünschenswert, eine Endoprothese bereitzustellen, die nur teilweise expandiert oder während des Einsetzens überhaupt nicht expandiert und die zu einem späteren Zeitpunkt nach der Einsetzung expandiert.
- Die vorliegende Erfindung liefert solch eine Endoprothese.
- Solange es nicht anderweitig angezeigt wird, soll für die Zwecke dieser Offenbarung der Begriff "Abbau" sich auf die übliche Bedeutung beziehen, ebenso wie biologischer Abbau, Erosion und Auflösung.
- Kurze Zusammenfassung der Erfindung
- Die Erfindung ist auf einen expandierbaren Stent gerichtet, der ein expandierbares Stentgerüst umfasst, das von einer Konfiguration mit vermindertem Durchmesser zu einer voll expandierten Konfiguration expandierbar ist. Zumindest ein den Stent haltendes Segment, das dafür konstruiert ist, auszufallen, ist um das Stentgerüst herum angeordnet, um das Stentgerüst solange in der Lage mit einem verminderten Durchmesser zu halten, bis das Haltesegment ausfällt. Das Haltesegment kann so gebaut sein, dass es ausfällt, nachdem eine vorherbestimmte Zeitspanne abgelaufen ist, und/oder, nachdem ein Druck, dessen Größe vorher festgelegt wurde, auf den Stent ausgeübt worden ist. Das Haltesegment kann vollständig biologisch abbaubar, zum Teil biologisch abbaubar, erodierbar oder aus einem inaktiven Material mit Ermüdungspunkten hergestellt werden.
- In einer Ausführungsform der Erfindung bildet das Haltesegment ein Band um die Außenseite des Stents herum, oder es ist um den Stent herum und mit ihm verflochten. Im Detail kann das Haltesegment so gewebt sein, dass es auf den inneren und äußeren Oberflächen der Streben alterniert.
- Das Haltesegment kann auch in Form eines Netzes bereitgestellt werden.
- Im Gebrauch wird der erfinderische Stent eingesetzt und expandiert weiter, nachdem das Haltesegment entfallen ist. Im Falle eines inaktiven Segments kann der Ausfall des Haltesegments von einem mechanischen Ausfall herrühren. Belastungen des Stents, die voraussichtlich zu dem gewünschten Ausfall führen oder zu ihm beitragen, schließen pulsierenden Blutdruck ein, der dafür gedacht ist, abwechselnd ein Verlängern und Verkürzen des Stents auszulösen.
- Im Falle eines biologisch abbaubaren Segments oder eines erodierbaren Segments, rührt der Ausfall aus dem Abbau oder der Erosion des Segments her.
- Im Falle der Herstellung eines Segments aus inaktivem Material und der Verbindungspunkte aus biologisch abbaubarem Material, resultiert der Ausfall letztendlich aus dem Abbau oder der Erosion der Verbindungspunkte.
- Allgemeiner ausgedrückt, ist die Erfindung auf den Einsatz eines sekundären erhöhten expansiven Drucks auf einen Körperhohlraum durch eine implantierbare, expandierbare medizinische Vorrichtung, beispielsweise Stent-Endoprothesen, einschließlich Stents, Stent-Implantate, Implantate und Hohlvenenfilter, zu einer vorherbestimmten Zeit nach der Implantation in einen Körperhohlraum, gerichtet.

- Obwohl diese Erfindung in vielen verschiedenen Formen ausgeführt werden kann, werden hierin im Detail bevorzugte spezielle Ausführungsformen der Erfindung beschrieben. Diese Beschreibung ist eine Erläuterung der Prinzipien der Erfindung, und es ist nicht beabsichtigt, die Erfindung auf die speziellen Ausführungsformen zu beschränken, die erläutert werden.
- Mit Bezugnahme auf die Figuren im Allgemeinen und auf
1 im Speziellen ist die vorliegende Erfindung auf einen expandierbaren Stent gerichtet, allgemein dargestellt unter100 , der das expandierbare Stentgerüst110 und die Stenthaltesegmente115 umfasst, die um ausgewählte Teile des Stentgerüsts herum angeordnet sind. Das expandierbare Stentgerüst110 ist ein selbstexpandierender Stent, der von einer Konfiguration mit vermindertem Durchmesser zu einer vollständig expandierten Konfiguration expandierbar ist. Die Stenthaltesegmente115 halten das Stentgerüst in einer weniger als vollständig expandierten Konfiguration, bis sie wegfallen. In der Ausführungsform der1 ist ein Strebenpaar120 pro Stentabschnitt125 mit einem Haltesegment115 verbunden. Jedes Stenthaltesegment115 ist ringsum vom benachbarten Stenthaltesegment durch eine Strebe120 seitlich versetzt. - Außerdem zieht die Erfindung in Betracht, eine Mehrheit von Stenthaltesegmenten pro Stentabschnitt vorzusehen.

Fachleute werden erkennen, das die Erfindung Stents in Betracht zieht, in denen eine gewünschte Anzahl von Stentabschnitten Stenthaltesegmente, wie in Abschnitt125a , besitzt. Ebenso zieht die Erfindung Stents in Betracht, die Stenthaltesegmente aufweisen, die gänzlich wie in Abschnitt125b angeordnet sind. Es wird außerdem erkennbar werden, dass die Erfindung Stents mit Stentabschnitten in Betracht zieht, die, wie in den Abschnitten125d und125e (dargestellt), miteinander verbunden sind. Schließlich wird erkennbar werden, dass jedes Haltesegment, über die beiden in der Figur dargestellten Streben hinaus, mehrere Streben zurückhalten kann. - Die Verwendung kleiner Haltesegmente ist aus verschiedenen Gründen vorteilhaft. Erstens ermöglichen individuelle Haltesegmente des Stents eine größere Flexibilität des Stents. Zweitens, führt der Gebrauch relativ kleiner Haltesegmente zu einer verminderten negativen Gewebereaktion auf das Implantat und speziell auf das Segment. Drittens, ermöglichen nur Stenthaltesegmente eine bessere Kontrolle über die Stentexpansion und folglich darüber, wie die zusätzliche sekundäre Kraft im Körperhohlraum angewendet wird.
- Die Erfindung zieht außerdem den Gebrauch größerer Stenthaltesegmente in Form von Stenthaltebändern in Betracht. In einer Ausführungsform der Erfindung bilden die Stenthaltesegmente ein Band oder mehrere Bänder, die sich um die Peripherie (des Stents) herum erstrecken. Der Stent der
4a , allgemein dargestellt unter100 , umfasst ein expandierbares Stentgerüst110 und ein Stenthaltesegment115 , das um die Peripherie des Stents110 herum in Form eines Bandes angeordnet ist. Das expandierbare Stentgerüst110 ist ein selbstexpandierender Stent, der von einer Konfiguration mit vermindertem Durchmesser zu einer vollständig expandierten Konfiguration erweiterbar ist. Die Stenthaltesegmente115 halten das Stentgerüst in einer weniger als vollständig expandierten Konfiguration, bis sie ausfallen. Außerdem zieht die Erfindung die Möglichkeit in Betracht, den Stent mit zusätzlichen Haltebändern, über die zwei in4a dargestellten Haltebänder hinaus, auszustatten. - Die bandähnlichen Segmente können sich um den Stent herum von der Außenseite des Stents her erstrecken, oder sie können um die Peripherie des Stents herum verflochten sein, wie es in den
4b und4c dargestellt wird. Die Bänder115 winden sich zwischen den Streben120 hindurch. - Die Erfindung zieht mehrere verschiedene Arten von Stenthaltesegmenten in Betracht. Ein zur Verwendung in der vorliegenden Erfindung passendes Stenthaltesegment wird in
5a dargestellt. Das Stenthaltesegment215 besitzt einen einzigen Ermüdungspunkt220 . Der Ermüdungspunkt220 besteht in einem verengten Bereich des Bandes.5b zeigt ein Stenthalteband215 mit zwei Ermüdungspunkten220 . Die Form der in den5a und5b dargestellten Ermüdungspunkte ist nur für Zwecke der Veranschaulichung vorgesehen. Ermüdungspunkte anderer Geometrien und Konfigurationen können ebenso verwendet werden. Ein Halteband mit einem einzigen Ermüdungspunkt kann ebenso benutzt werden wie ein Halteband mit zusätzlichen Ermüdungspunkten.5c zeigt ein Haltesegment215 mit zwei Löchern223 , die aus dem Haltesegment herausgestanzt sind. In diesem Bereich können weniger Löcher oder weitere Löcher ebenso vorhanden sein, um das Segment zu schwächen. - Das Halteband
215 ist aus inaktivem oder biologisch stabilem Material, beispielsweise Polytetrafluorethylen (PTFE), hergestellt. Andere inaktive oder biologisch stabile Materialien können ebenfalls benutzt werden. Wünschenswerterweise wird ein Material mit einer geringen Gewebereaktion, zum Beispiel PTFE, und werden Materialien mit einer geringen Gewebereaktion, wie Polyurethane, Polyester, und Silikone verwendet. - Ein anderes geeignetes Stenthaltesegment wird in allgemein üblicher Form unter
315 in6a dargestellt. Das Haltesegment315 wird von zwei Segmenten aus inaktivem oder biologisch stabilem Material325 und einem Segment aus biologisch abbaubarem Material320 gebildet, die zusammengefügt werden. Eine Bauart wie diese kann auch für ein Stenthalteband, wie in6b dargestellt, verwendet werden. Das Halteband315 wird aus einem Segment aus inaktivem oder biologisch stabilem Material325 und einem Segment aus biologisch abbaubarem Material320 gebildet, wobei die Segmente zusammengefügt werden. Wie in den6a und6b dargestellt, wird das biologisch abbaubare Segment320 in Gestalt eines Punktes bereitgestellt. Das biologisch abbaubare Segment320 kann auch in anderen Formen vorgesehen werden. Insbesondere kann das Haltesegment oder Halteband in derselben allgemeinen Form wie die in den5a und5b dargestellten Bänder215 bereitgestellt werden, wobei der Ermüdungspunkt durch ein biologisch abbaubares Segment da ersetzt wird, wo sich das Band verengt. In6c wird eine Ausführungsform des Haltebands315 dargestellt, das zwei inaktive Segmente325 und zwei biologisch abbaubare Segmente320 besitzt. Ein Haltesegment kann in gleicher Weise mit mehreren biologisch abbaubaren Segmenten ausgerüstet werden. Außerdem zieht die Erfindung die Verwendung zusätzlicher inaktiver Segmente und zusätzlicher biologisch abbaubarer Segmente in Betracht. - Eine andere Ausführungsform der Erfindung wird in
7 dargestellt. Der Stent100 umfasst ein expandierbares Stentgerüst110 und ein Stenthalteband115 , das über dem Stent um die Peripherie des Stents110 herum angeordnet ist. Das expandierbare Stentgerüst110 ist ein selbst-expandierender Stent, der sich von einer Konfiguration mit vermindertem Durchmesser zu einer vollständig expandierten Konfiguration erweitern kann. Das Stenthalteband115 ist um das Stentgerüst110 herum unter einem schrägen Winkel relativ zur Längsachse130 des Stents angeordnet. Es ist nicht beabsichtigt, die Erfindung auf den dargestellten spezifischen schrägen Winkel zu beschränken, stattdessen ist aber beabsichtigt, die volle Variationsbreite der schrägen Winkel einzuschließen. Der Stent kann weniger oder mehr Haltebänder aufweisen, als in7 dargestellt wird. - Eine andere Ausführungsform wird in
8 dargestellt. Der Stent100 wird in einer, Konfiguration mit vermindertem Durchmesser mittels Stenthalteband115 gehalten, das spiralförmig um den Stent herum angeordnet ist. - Die Erfindung zieht ebenfalls die Verwendung von Haltestrukturen, zum Beispiel von gewebten Strukturen oder Netzen, in Betracht, um den Stent in einer reduzierten Konfiguration oder in einer weniger als gänzlich expandierten Konfiguration zu halten. Im Allgemeinen besteht die gewebte Struktur aus einem verflochtenen oder geschlossenen Zelldesign, das mit der Außenseite des Stents verschweißt ist. Wie es allgemein in
11a unter415 dargestellt wird, kann die gewebte Struktur die Form eines Rohres besitzen, das einzelne Segmente des Stents umhüllt. Wie in11b dargestellt wird, kann die gewebte Struktur auch die gesamte Länge des Stents umschlingen. Die gewebte Struktur415 ist um die Länge des Stentgerüsts110 herum angeordnet. Wünschenswerterweise besitzt die gewebte Struktur Wegfallpunkte420 , die es der gewebten Struktur ermöglichen, nach dem Ausfall auseinander zu reißen. - Wie in
11c unter415 dargestellt, kann die gewebte Struktur auch ein flache Folie sein. Die gewebte Struktur415 bedeckt eine Gruppe von Zellen wie ein Lappen. Die gewebte Lappenstruktur kann entweder Wegfallpunkte, zum Beispiel Ermüdungspunkte, oder abbaubare Teile aufweisen, um den Stent freizusetzen. Wie in11d dargestellt, kann die gewebte Lappenstruktur an den Stentstreben mittels eines abbaubaren Materials befestigt werden. Die gewebte Struktur415 ist am Stent100 mittels des abbaubaren Bindemittels122 befestigt. - Eine andere Konfiguration der gewebten Struktur wird in
11e unter415 dargestellt. Die gewebte Struktur415 besteht aus einem hexagonalen Muster mit Ermüdungspunkten420 an den Eckpunkten der Hexagone. Eine polygonale Konfiguration kann gegen eine andere polygonale Konfiguration ausgetauscht werden. - So wie die Haltesegmente kann die gewebte Struktur aus einem biologisch abbaubaren Material oder einem inaktiven Material mit Ermüdungspunkten hergestellt werden. Ein inaktives Material mit biologisch abbaubarem verbindendem Material kann ebenfalls verwendet werden.
- Obwohl hierin spezielle Konfigurationen gewebter Strukturen dargestellt worden sind, werden Fachleute diese als beispielhaft verstehen. Andere Konfigurationen gewebter Strukturen können auch verwendet werden.
- Haltesegmente in der Form dünner schmaler Streifen können aus den Folien eines gewünschten Materials ausgeschnitten werden. Auch können die Fäden eines Haltesegments durch Extrusion hergestellt werden. Ermüdungspunkte werden dadurch hergestellt, dass die Extrusionsrate des Fadens variiert wird oder die Spannkraft des Fadens während der Extrusion variiert wird. Die Spannkraft kann außerdem verwendet werden, um Ermüdungspunkte in andere Arten von Haltesegmenten, zum Beispiel in bandförmige Haltesegmente, einzuarbeiten. Materialien, wie zum Beispiel PTFE, werden sich unter Spannung einschnüren und dadurch einen Ermüdungspunkt bilden. Wie in
5c veranschaulicht, können Ermüdungspunkte auch dadurch im Material platziert werden, dass das Material mit einem Laser oder einer Stanze ausgeschnitten wird. Andere Verfahren zur Einarbeitung von Ermüdungspunkten in ein Haltesegment umfassen die punktförmige Anwendung eines Lasers, die Anwendung von Erwärmung, von einem oder mehreren Lösungsmitteln oder von anderen Chemikalien, zum Beispiel Ätzmitteln, oder die Anwendung von radioaktiver Strahlung, um das Material zu schwächen oder um das Material brüchig zu machen. Ermüdungspunkte können außerdem durch die Verwendung eines zweiten Materials im Segment in ein Haltesegment eingearbeitet werden. - Wie in einigen der Figuren dargestellt wird, ist das Haltesegment auf der Außenseite des Stents angeordnet. Vor Ort ist das Halteband zwischen der Gefäßwand und dem Stent angeordnet, es erleichtert dadurch die Einkapselung des Haltebands im Gewebe und verhindert, dass das inaktive Material im Gefäß davongetragen wird. Dies ist dort besonders wünschenswert, wo inaktives Material für das Halteband verwendet wird.
- Das Haltesegment kann auf vielfältigen Wegen am Stent befestigt werden. Fadenähnliche oder andere Haltesegmente im passenden Format können durch den Stent hindurch verflochten sein und/oder mit dem Stent oder mit den Stentstreben verbunden sein.
- Wie in
9 dargestellt, kann ein Stück des Haltesegments115a wahlweise auf einer äußeren Oberfläche einer Strebe120a platziert werden, und ein anderes Stück des Haltesegments115b kann auf der inneren Oberfläche einer Strebe120b platziert werden. Die zwei Stücke können dann bei121 miteinander durch Wärme, Lösungsmittel oder Bindemittel vereinigt werden. - Andere Befestigungsverfahren umfassen das Ausbilden des Haltesegments als Schleife um eine Strebe herum und die Befestigung des Segments an sich selbst oder durch Verschmelzen mit der Strebe durch Wärme oder Lösungsmittel oder Bindemittel. Das Haltesegment kann auch um die Streben herum gewickelt oder um die Streben herum beschichtet oder unmittelbar an den Streben befestigt werden und dort durch die Anwendung von Wärme oder von einem Lösungsmittel oder von anderen geeigneten Bindemitteln, einschließlich biologisch resorbierbarer, biologisch abbaubarer oder erodierbarer Zusammensetzungen, zum Beispiel solcher, die unten offengelegt werden, befestigt werden. Wie in den
4c und10 dargestellt, kann auch ein biologisch stabiles Material mit der Strebe oder um die Strebe herum an einzelnen Punkten verschmolzen werden und dann mit einem aus biologisch abbaubarem Material hergestellten Haltesegment115 durch Wärme, Lösungsmittel oder Bindemittel vereinigt werden. Da, wo ein Lösungsmittel benutzt wird, kann das überschüssige Lösungsmittel verdampft oder auf andere Weise beseitigt werden. - Eine Befestigungsmethode umfasst die Bereitstellung eines Mantelrohres mit darauf punktförmig angebrachtem abbaubarem Material. Sodann wird ein Stent um das Mantelrohr herum angeordnet und ein Streifen aus PTFE über einem Teil des Stents platziert. Gewünschte Teile des Stents können so befestigt werden, dass der Stent in einer reduzierten Konfiguration auf dem Mantelrohr festgehalten wird. Dem Stent wird Wärme zugeführt, um die Punkte mit dem Stent und dem PTFE zu verschmelzen. Der Stent wird dann abgekühlt und vom Mantelrohr entfernt.
- In einem anderen Verfahren zur Anbringung eines Haltesegments an einem Stent, werden die Haltesegmente in einer Modellform platziert, die sodann um einen Stent mit einer Konfiguration mit vermindertem Durchmesser herum angeordnet wird. Dann wird der Stent ausgeweitet. Speziell bei einem gewünschten mittleren Durchmesser des Stents wird eine ringförmige Modellform bereitgestellt. Die Modellform wird in zwei Hälften geliefert, von denen wird die eine unter
500 in12 dargestellt, mit Vertiefungen501 , die aus der Oberfläche des inneren Durchmessers für das Muster des Haltesegments ausgeschnitten wurden. Die Haltesegmente aus einem gewünschten Material werden in den Vertiefungen in den Modellformhälften platziert und die Modellformhälften wieder montiert. Ein Mantelrohr wird bereitgestellt, und ein Nitinol-Stent wird unter die austenitischemartensitische Übergangstemperatur abgekühlt, um ihn martensitisch zu machen, und auf dem Mantelrohr in seiner Größe zu reduzieren. Wahlweise kann das Mantelrohr Vertiefungen aufweisen, die dort im Muster des Haltesegments ausgeschnitten sind, um ein gewünschtes Material auf dem inneren Durchmesser des Stents zu platzieren. Der reduzierte Stent und das reduzierte Mantelrohr werden in der Modellform platziert. Der Stent wird dann erwärmt, um innerhalb der Modellform zu expandieren. Bindemittel, Wärme oder Lösungsmittel werden dann eingesetzt, um die Haltesegmente miteinander und/oder mit den Stentstreben zu vereinigen, und um den Stent auf dem gewünschten Durchmesser zu fixieren. - In noch einem anderen Verfahren der Anbringung eines Haltesegments an einem Stent können Clips verwendet werden, um Streben oder Zellen in der geschlossenen Position in einem gewünschten Teil oder in mehreren gewünschten Teilen des Stents zu halten. Fäden aus dem Material der Haltesegmente werden über diesen Teilen des Stents angeordnet und an den Streben befestigt, wobei Bindemittel, Wärme oder Lösungsmittel benutzt werden.
- Haltesegmente können auch durch Haltepunkte aus warm extrudiertem Material zwischen bestimmten Streben am Stent (beispielsweise Streben in geschlossener Position oder in einer Position mit vermindertem Durchmesser) befestigt werden.
- Haltesegmente können auch eingesetzt werden, um den Stent zuerst durch die Benutzung einer Halteform oder die Benutzung von Klemmbefestigungen in einer Konfiguration mit gewünschtem vermindertem Durchmesser zu halten. Die Fäden des haltenden Materials können sodann auf die Innenseite des Stents oder auf die Außenseite des Stents in dem Bereich gesprüht werden, der festgehalten werden soll.
- Die
13a –13d veranschaulichen einen Teil des Stents mit einem Haltesegment vor und nach dem Ausfall des Haltesegments. Wie in den13a und13c dargestellt wird, besitzt der Stent110 ein Haltesegment115 , das zumindest einen Teil des Stents in einer reduzierten Konfiguration festhält. Das Segment115 ist an den verschiedenen Punkten122 am Stent befestigt. Wie in den13b und13d dargestellt wird, expandiert der Stent110 nach dem Ausfall des Haltesegments115 . - Geeignete biologisch abbaubare oder biologisch resorbierbare Materialien zur Verwendung in Haltesegmenten, Bändern oder gewebten Strukturen umfassen:
Poly(L-lactide) (PLLA), Poly(D,L-lactide) (PLA), Poly(glycolide) (PGA), Poly(L-lactide-co-D, L-lactide) (PLLA/PLA), Poly(L-lactide-co-glycolide) (PLLA/PGA), Poly(D,L-lactide-co-glycolide) (PLA/PGA), Poly(glycolide-co-Trimethylencarbonat) (PGA/PTMC), Polydioxanon (PDS), Polykaprolakton (PCL), Polyhydroxybutyrat (PHBT), Poly (Phosphazene) Poly(D,L-lactide-co-Kaprolakton) (PLA/PCL), Poly(glycolide-co-Kaprolakton) (PGA/PCL), Poly(Phosphatester), Poly(DL-Milchsäure), Poly(Glykolsäure). - PGA und PLLA/PGA sind besonders wünschenswerte Materialien. Andere geeignete Materialien umfassen Kollagen, Fibrin und Zellulose.
- Noch allgemeiner ausgeführt, umfassen die üblichen Kategorien von abbaubarem Material Polyester, Polyamide, Polyanhydride und Polyorthoester. Die zwei Letzteren sind an der Oberfläche erodierbare Arten. Alle diese Materialien sind nur beispielhaft aufgeführt.
- Wie oben erörtert, können die Haltesegmente so konstruiert werden, dass sie für eine vorherbestimmte Zeitspanne vor ihrem Ausfall erhalten bleiben. Da, wo das Haltesegment biologisch abbaubare Materialien enthält, kann die Lebensdauer des Segments dadurch verändert werden, dass die chemische Struktur des biologisch abbaubaren Teils des Segments verändert wird. Die Hydrolyse ist die grundlegende Reaktion der meisten biologisch abbaubaren Polymere. Die hydrolytische Abbaurate kann einige tausendfach dadurch verändert werden, dass die chemische Struktur im Polymer-Basisnetz geändert wird. Zum Beispiel bauen fetthaltige Polyanhydride in wenigen Tagen ab, während die aromatischen Polyanhydride über einen Zeitraum von wenigen Jahren zerfallen. Polyorthoester ist ein langsam an der Oberfläche erodierendes Material. In Gegenwart eines Säurezusatzes, eines sogenannten Hilfsstoffs, besitzt es eine schnellere Abbaurate. Dagegen unterdrückt es den Abbau in Gegenwart einer alkalischen Substanz. Wenn länger haltende Haltesegmente gewünscht werden, werden das aromatische Polyanhydrid und das nicht-additive Polyorthoester die bevorzugten Segmentmaterialien sein.
- Außerdem können beispielsweise Polymere, die hydrolytisch labile Verbindungen in ihrem Basisnetz enthalten, durch zwei verschiedene Mechanismen hydrolisieren. Diese Mechanismen sind die Masseerosion und die Oberflächenerosion. In einem masseerodierenden Polymer tritt der hydrolytische Prozess überall auf der Matrix des Polymers auf, wohingegen die Hydrolyse bei der Oberflächenerosion nur auf die äußere Schicht des Polymers beschränkt bleibt. Folglich ist die Letztere besonders bevorzugt, wenn ein längerer Abbau gewünscht wird.
- Spezielle Segmentmaterialien umfassen beispielsweise Poly(orthoester), wie 50:50 HD/t-CDM (1,6-Hexanediol-co-trans-Cyclohexanedimethanol) Poly(orthoester) mit 0.2% Poly(Sebacinanhydrid) als Hilfsstoff. Polyanhydride, wie Poly[bis(p-carboxyphenoxy)Propananhydrid] (PCPP) und Poly(terephtalisches Anhydrid) (PTA) können ebenfalls verwendet werden.
- Wenn ein Polyanhydrid als Segmentmaterial ausgewählt wird, kann zum Beispiel seine Dicke ausgewählt werden, um die Abbaudauer zu steuern. Somit kann ein Segment so konfektioniert werden, dass es eine Abbaudauer von einer Woche, von einem Monat, von zwei Monaten, oder von bis zu sechs Monaten, oder von jeder anderen geeigneten Zeitspanne, aufweist. Zum Beispiel kann ein Segmentmaterial aus PGA mit so einer ausgewählten Dicke angeboten werden, dass eine Abbaudauer von beispielsweise zwei Wochen zur Verfügung steht.
- Das Haltesegment ist wünschenswerterweise so konstruiert, dass es nach einem vorherbestimmten Zeitraum im Anschluss an die Implantation in den Körperhohlraum ausfällt. Der Ausfall beinhaltet mechanischen Ausfall und den Ausfall, der aus dem biologischen Abbau von zumindest einem Teil des Segments herrührt. Wünschenswert sind Haltesegmente, die nach ungefähr einer Woche im Hohlraum ausfallen, wie es Rückhaltesegmente sind, die nach ungefähr einem Monat im Hohlraum ausfallen. Noch wünschenswerter ist es, dass die Rückhaltesegmente von etwa einem Monat bis zu etwa sechs Monaten im Anschluss an die Implantation halten. Dies ermöglicht die sichere Anwendung von zusätzlichem Druck auf den Körperhohlraum nach der ursprünglichen Gewebereaktion auf das Implantat und nachdem sich die Umformung des Hohlraums fortentwickelt hat, oder zu einer Zeit, in der ein zusätzlicher externer Druck auf den Körperhohlraum erwartet wird, wie im Fall eines benachbarten wachsenden Tumors.
- Die Haltesegmente können auch so konstruiert werden, dass sie nach der Anwendung von Druck in einem vorher festgelegten Umfang, wie zum Beispiel durch einen Ballon, ausfallen.
- Die Erfindung zieht außerdem in Betracht, das Haltesegment mit einem Behandlungsmittel oder einem therapeutischen Wirkstoff anzubieten. Ein Material für ein Haltesegment, das den therapeutischen Wirkstoff eluiert, kann durch Lösung oder Auflösung des(r) Wirkstoffs(e) und des Polymers des Haltesegments in einem Lösungsmittel hergestellt werden. Sodann können Fäden extrudiert und Beläge gesprüht oder gegossen werden, um Streifen oder gewebte Strukturen zur Verwendung als Haltesegmente herzustellen. Das Behandlungsmittel kann auch durch alle anderen geeigneten Techniken, einschließlich der Imprägnierung oder der Beschichtung des Segments mit dem Behandlungsmittel zur Verfügung gestellt werden.
- Wünschenswerte Arten therapeutischer Wirkstoffe umfassen entzündungshemmende Wirkstoffe, wucherungshemmende Wirkstoffe, Anti-Blutplättchen (anti-platelet) Wirkstoffe, Anti-Thrombin-Wirkstoffe, Wirkstoffe der Gentherapie und geeignete Kombinationen der obengenannten Wirkstoffe.
- Geeignete Behandlungsmittel umfassen Medikamente, zum Beispiel radiochemische Medikamente zum Bestrahlen und zur Verhinderung von Gewebewachstum und von menschlichen Wachstumsfaktoren, einschließlich VEGF (Vaskulärer Endothel Wachstums-faktor), TGF-beta (Transformierender Wachstumsfaktor-Beta), IGF (Insulinähnlicher Wachstumsfaktor), PDGF (Thrombozyt – abgeleiteter Wachstumsfaktor) und FGF (Fibroblast Wachstumsfaktor) etc. Das Medikament kann blutgerinnungshemmend sein, zum Beispiel D-Phe-ProArg Chloromethylketon. Eine RGD (Arginin-Glycin-Asparaginsäure) Peptid-enthaltende Verbindung, Heparin, Antithrombin-Verbindungen, Thrombozyt-Rezeptor-Antagonisten, Antikörper, Aspirin, Prostaglandin-Hemmstoffe, Thrombozyt-Hemmstoffe, oder Anti-Thrombozyt-Peptid können ebenfalls verwendet werden.
- Das Medikament kann ein Hemmstoff gegen das vaskuläre Zeltwachstum, DNA, RNA, cholesterinsenkende Wirkstoffe, gefäßerweiternde Wirkstoffe sein. Andere geeignete Medikamente umfassen TaxolTM (paclitaxel), 5-Fluorourazil, Beta-Estradiol. Metaboliten und andere Derivate der obengenannten Medikamente oder Verbindungen können ebenfalls eingesetzt werden. Jedes Behandlungsmittel kann selbständig oder in Verbindung (mit anderen Mitteln) benutzt werden.
- Das Behandlungsmittel kann in die Haltesegmente imprägniert werden oder es kann als ein Überzug auf den Segmenten zur Verfügung gestellt werden. Das Behandlungsmittel kann durch Eintauchen der Segmente in das Behandlungsmittel oder durch Sprühen des Mittels auf die Segmente aufgetragen werden. Geeignete Techniken für das Auftragen des Behandlungsmittels können in der US Patentanmeldung 08/874190 gefunden werden, auf die hier Bezug genommen wird und die hier in ihrer Gesamtheit eingefügt ist.
- Die Erfindung ist außerdem auf einen erweiterbaren Stent zur Implantation in einen Körperhohlraum gerichtet, der ein erweiterbares Stentgerüst und ein Stenthaltesegment umfasst, das für den Ausfall konstruiert ist. Wie oben erörtert wurde, kann das Haltesegment so gestaltet werden, dass es nach einer vorherbestimmten Zeitspanne, zum Beispiel etwa einer Woche oder etwa einem Monat, ausfällt. Das Haltesegment kann auch so gestaltet werden, dass es auf die Ausübung eines vorherbestimmten Drucks hin, der zum Beispiel von einem Ballon geliefert wird, ausfällt.
- Die erfinderischen Stents, Implantate und Stentimplantate können aus jedem in Fachkreisen bekannten geeigneten selbst-expandierenden und ballonunterstützt selbst-expandierenden Stent oder Implantat gebildet werden. Solch ein geeigneter selbst-expandierender Stent wird in der allgemein so bezeichneten US Patentschrift No. 08/511076, angemeldet am 3. August 1995, offenbart. Ein anderer geeigneter Stent ist der Wallstent Stent, beschrieben in
US 4,655771 ,US 4,954,126 undUS 5,061,275 . - Geeignete Implantate können dadurch gebildet werden, dass ein Implantatmaterial, zum Beispiel Polytetrafluorethylen (PTFE) oder Polyethylenterephthalat (PET), auf den Stent aufgetragen und dort durch geeignete Mittel, wie chirurgische Nähte, befestigt wird. Andere Implantatmaterialien einschließlich Polyurethan, Kollagen, Silikon, Polypropylen, und Polyolefin können ebenfalls verwendet werden. Das Implantatmaterial kann als inneres Rohr für den Stent oder als äußeres Rohr für den Stent verwendet werden. Andere Arten selbst-expandierender Stents können ebenfalls bei der Anwendung der Erfindung eingesetzt werden.
- Es ist beabsichtigt, die Erfindung mit den üblichen selbst-expandierenden Endoprothesen zu verwenden, und als solche bleibt sie nicht auf die koronaren Endoprothesen beschränkt. Nicht-koronare Anwendungsfälle umfassen Gallenprothesen und Harnröhrenprothesen. Speziell die erfinderischen Endoprothesen können sich dort als nützlich erweisen, wo es notwendig ist, die Durchgängigkeit von Gefäßen und Kanälen aufrechtzuerhalten, die im Begriff sind, von Tumoren geschlossen zu werden, einschließlich der Tumore des Gallengangs und der Bauchspeicheldrüse.
- Die Erfindung sieht Endoprothesen vor, die während der Anwendung teilweise expandieren können oder überhaupt nicht expandieren können, und die noch zu einem späteren Zeitpunkt infolge des Ausfalls eines Haltesegments expandieren.
- Ein expandierbarer Stent mit Haltesegmenten, wie sie oben offengelegt wurden, kann in einen Körperhohlraum implantiert werden. Die Haltesegmente können so konstruiert sein, dass sie nach einer vorherbestimmten Zeitspanne, wie zum Beispiel einer Woche, einem Monat oder länger, durch biologischen Abbau oder Erosion oder mechanischen Defekt ausfallen. Der Stent wird in einem weniger als vollständig expandierten Zustand implantiert und expandiert nach dem Durchgang nach einer vorherbestimmten Zeitspanne, zum Beispiel einer Woche, einem Monat oder länger mit dem Ausfall oder der Auflösung der Haltesegmente. Der Stent kann an die gewünschte Körperstelle transportiert werden, unter Verwendung eines Stent transportierenden Katheters, wie zum Beispiel solcher Katheter, wie sie in
US 5,534,007 undUS 5,571,135 offen gelegt wurden, oder irgendeines anderen geeigneten Stent transportierenden Katheters. - Wünschenswerterweise werden die Haltesegmente so konstruiert werden, dass sie ausfallen, nachdem das Gefäß ausreichend ausgeheilt ist, um im Stande zu sein, der Druckerhöhung nach dem Ausfall der Segmente sicher standzuhalten.
- Ein Stent mit Haltesegmenten, wie sie oben beschrieben wurden, kann in einen Körperhohlraum eingesetzt werden, und ein vorherbestimmter Druck kann auf den Stent ausgeübt werden, um den Stent vollständiger zu erweitern. Ein zusätzlicher Druck kann sofort nach der Entfaltung des Stents als eine "Ausbesserung" oder nach einer geeigneten Zeitspanne (beispielsweise Tage, Wochen oder Monate) im Anschluss an die Entfaltung ausgeübt werden.
- Die Erfindung ist außerdem allgemeiner auf eine expandierbare medizinische Vorrichtung gerichtet, die durch eine oder mehrere Haltestrukturen, wie die oben beschriebenen Segmente, in einem zusammengezogenen oder teilweise zusammengezogenen Zustand gehalten wird. Geeignetes medizinisches Gerät umfasst Stents, Stent-Implantate, Implantate und Hohlvenenfilter.
Claims (22)
- Ein expandierbarer Stent (
100 ), der umfasst: ein expandierbares Stentgerüst (110 ), wobei das Stentgerüst von einer Konfiguration mit reduziertem Durchmesser zu einer vollständig expandierten Konfiguration ausdehnbar ist; und zumindest ein Stenthaltesegment (115 ) um das Stentgerüst herum angeordnet ist, wobei das Stenthaltesegment (115 ) das Stentgerüst (110 ) in einer geringer als voll expandierten Konfiguration aufrechterhält, dadurch charakterisiert, dass das Stenthaltesegment so konstruiert und angeordnet ist, dass es nach dem Abbau von zumindest einem Teil des Segments ausfällt. - Der Stent nach Anspruch 1, der eine Vielzahl von Stenthaltesegmenten (
115 ) aufweist. - Der Stent nach Anspruch 1, wobei das Haltesegment (
315 ) aus zumindest einem Segment aus inaktivem Material (325 ) und zumindest einem Segment aus biologisch abbaubarem Material (320 ) gebildet wird, die miteinander verbunden sind. - Der Stent nach Anspruch 3, wobei das Haltesegment (
315 ) eine Vielzahl von alternierenden inaktiven Segmenten (325 ) und biologisch abbaubaren Segmenten (320 ) umfasst, die miteinander verbunden sind. - Der Stent nach Anspruch 1, wobei das Haltesegment (
315 ) aus biologisch abbaubarem Material hergestellt ist. - Der Stent nach Anspruch 5, wobei das biologisch abbaubare Material mit einem Behandlungsmittel bereitgestellt wird.
- Der Stent nach Anspruch 1, wobei das Haltesegment (
115 ) mit einem Behandlungsmittel bereitgestellt wird. - Der Stent nach Anspruch 1, wobei das Haltesegment (
115 ) auf der Außenseite des Stentgerüsts (110 ) angeordnet ist. - Der Stent nach Anspruch 1, wobei das Haltesegment (
115 ) durch das Stentgerüst (110 ) hindurch und mit ihm verflochten ist. - Der Stent nach Anspruch 1, wobei das Haltesegment (
115 ) so konstruiert ist, dass es nach einer vorherbestimmten Zeitspanne im Körper ausfällt. - Der Stent nach Anspruch 10, wobei die Zeitspanne zumindest einen Monat umfasst.
- Der Stent nach Anspruch 10, wobei das Haltesegment (
115 ) in Form eines Bandes um das Stentgerüst (110 ) herum angeordnet ist. - Der Stent nach Anspruch 1, wobei das Stentgerüst (
110 ) eine Vielzahl von miteinander verbundenen Streben (120 ) umfasst und das Haltesegment (115 ) nicht mehr als zwei benachbarte Streben verbindet. - Der Stent nach Anspruch 1, wobei das Haltesegment (
115 ) die Form einer gewebten Struktur (415 ) besitzt. - Ein expandierbarer Stent (
100 ), der umfasst: ein expandierbares Stentgerüst (110 ), wobei das Stentgerüst von einer Konfiguration mit reduziertem Durchmesser zu einer vollständig expandierten Konfiguration ausdehnbar ist; und zumindest ein Stenthaltesegment (115 ) um das Stentgerüst herum angeordnet ist, wobei das Stenthaltesegment (115 ) das Stentgerüst (110 ) in einer geringer als voll expandierten Konfiguration aufrechterhält, dadurch charakterisiert, dass das Stenthaltesegment so konstruiert und angeordnet ist, dass es zumindest einen Ermüdungspunkt (220 ) aufweist. - Der Stent nach Anspruch 15, wobei das Haltesegment (
115 ) eine Vielzahl von Ermüdungspunkten (220 ) aufweist. - Der Stent nach Anspruch 15, wobei das Haltesegment (
115 ) sich am Ermüdungspunkt (220 ) verengt. - Der Stent nach Anspruch 15, wobei das Haltesegment (
115 ) aus einem inaktiven Material hergestellt ist. - Der Stent nach Anspruch 18, wobei das Material PTFE ist.
- Der Stent nach Anspruch 15, wobei das Stenthaltesegment (
115 ) so konstruiert ist, dass es nach einer vorherbestimmten Zeitspanne in einem Körperhohlraum ausfällt. - Der Stent nach Anspruch 15, wobei das Haltesegment (
115 ) so konstruiert ist, dass es zumindest nach einem Monat in einem Körperhohlraum ausfällt. - Der Stent nach Anspruch 15, wobei das Stenthaltesegment (
115 ) so konstruiert ist, dass es nach der Ausübung einer vorherbestimmten Kraft auf das Stenthaltesegment ausfällt.
