The catheter (12) extends through an external connector (14) and the connector end cap (15) and the free end of the catheter is clamped to a slider (17) which can slide on the guide tube. By moving the slider (17) along the guide tube (11) the catheter can be moved axially relative to the guide tube in the direction opposite to the direction of movement of the slider. USE/ADVANTAGE - For carrying out operations on the gall bladder under X-ray observation. The movement of the inner end of the catheter is accurately controlled by the slider (17).

Description

The invention is based on a surgical instrument, consisting of an insertion tube, one in the insertion tube displaceable in the axial direction and in the radial direction rotatable catheter and encased from a holding means for Moving the catheter in the insertion tube.

Such an instrument is from the book "Laparoscopic Biliary Surgery, "Blackwell Scientific Publications, Osney Mead, Oxford, 1990, page 62 ff known (Cholangiography cannula developed by Olsen). The well-known instrument is used for laparoscopic Cholecystectomy used.

Laparoscopic cholecystectomy is still a relatively young one minimally invasive surgical procedure. As with the conventional intervention (open, conventional cholecystecto mie), who has been practicing for more than a hundred years is also in the introduction of endoscopic Cholezys tectomy technique the discussion about intraoperative x-rays A new biological representation of the biliary tract emerged. While the intraoperative x-ray control of the biliary tract in the con conventional surgery has long been carried out routinely, she has been using laparoscopic technique because of technical Difficulties and initial low success rates (<50%) not yet fully implemented. The exploration of the Ductus cysticus is technically more difficult and therefore also more time consuming. On the other hand, there is no laparoscopic technique the exact representation of the cysticus confluence, the common bile duct becomes, if at all, from seen from a different perspective. It follows from this the need, in principle, by means of x-ray Representation of an exact topographical assignment of the extra and also to achieve intrahepatic bile duct system.

A catheter is intrapoerative in the cystic duct bring in. Once the catheter is in place, you can Contrast agent injection a representation of the extra and also Track intrahepatic bile duct system. On the cys  To this end, a small incision is made via which the Cysticus catheter is inserted. The threading of the Ka theters in the cystic duct can be particularly troublesome, so that instruments are needed for this process are particularly suitable.

In the known instrument (Cholangiography cannula developed by Olsen) are basket-like and looped at the end close to the body Shaped gripper designed to the bent in the tip Hold the catheter in place if the leads to the Gripper ends are operated accordingly. The leads too the gripper ends are through the metal insertion tube carried out. A self-retaining stable position However, the tip of the catheter is only conditional with this instrument possible. Furthermore, the gripper ends as well as the metal one Insertion tube with its radiopaque metal structure visual assessment of the course of the cystic duct disruptive.

Anatomical features, such as B. aberrating or acute This makes it easy to overlook sorean courses of the cysticus the risk of major complications from laparoscopic Cholecystectomy, namely the common bile duct injury latent further.

The instruments become direct during laparoscopic interventions moved, d. H. Leverage laws and fictional pivot points are to be make sure that the instruments are used for correct guidance can be. In the well-known instrument (cholangiography cannula developed by Olsen) is the handling, as already above shown, difficult.

The invention is therefore based on the object of an instrument of the type mentioned in such a way that the Movable ends of the instrument are reproducible and safe can be moved and that the radiological overview is not additionally disturbed by X-ray contrast.

This object is achieved in that the Holding means arranged on the distal end of the insertion tube and that the insertion tube is X-rayed casual material is made.

The instrument according to the invention thus has the essential Advantage that the catheter is fixed to the insertion tube no longer immediately before the entrance to the duct cysticus occurs but outside the patient. The field of vision for the surgeon is expanded, and in addition either Entry tube reduced in diameter, or in diameter larger catheters can be used.

Is the holding means designed as a control slide, which in the movement self-locking on the outer surface of the insertion tube is attached, can by shifting the position Control carriage also a safe and predictable position change in the catheter tip. The catheter tip is preferably curved as a cystic catheter. One in the longitudinal axis of the cystic catheter sliding guidewire, the beyond the centrally open tip of the cystic catheter protrudes, facilitates intubation into the cystic duct.

The control slide can only be overcome by overcoming one given frictional force can be shifted or rotated. These Measure closes an uncontrolled and unwanted situation  change of the catheter tip. By means of the possible axial and radial movement of the control carriage is a simple one and safe threading of the catheter into the cystic duct possible.

A lug is preferably formed on the control slide protrudes radially from the insertion tube. The location of the nose shows the angular position of the catheter tip in the patient's body. The cystic catheter can be attached to the nose. Will the Nose turned clockwise, so the catheter also turns peak clockwise. The nose turns counterclockwise rotated, the catheter tip follows this movement.

The control slide must also be designed so that it is off there is an inner ring that is only axially displaceable and moreover still guided in a groove of the insertion tube can be. The inner ring is surrounded by an outer ring, which is fixed immovably on the inner ring. The outside ring is rotatable in this embodiment around the inner ring. The outer ring can be next to a nose that connects the catheter to the outside holds detachable area, have notches that with recesses on Interact inner ring, so that from locking to locking a defined degree of rotation of the catheter tip takes place.

The nose of the control slide points to fix the catheter at the free end of an open slot through which a catheter can be pressed into the holding position. Through the slot the catheter can only slide when a certain detention friction value is exceeded.

In a further embodiment of the surgical instrument according to the invention, a connector is formed on the end of the insertion tube remote from the body, which connector can be closed by a cover cap. The cover cap is provided with an opening through which the catheter can be passed. Depending on the diameter of the catheter, different cover caps made of an elastomer material with openings of different sizes are used. The cover cap also acts as a septum and thus prevents CO 2 from escaping from the abdomen.

The insertion tube is preferably made of a transparent dimensionally stable plastic. Over the axial length of the Insertion tube and in the circumferential direction are markings provided, over which the control slide can be pushed. Due to the position of the control carriage to the markings the position of the catheter tip inside the body can be determined teln.

The surgical instrument according to the invention for roentgenolo representation of the intra- and extrahepatic biliary tract in laparoscopic cholecystectomy thus fulfills everyone extended requirements for successful probing of the cystic duct and necessary for contrast medium application are. The insertion tube is not radiopaque, and via the now easy and secure access to the Bile duct system can intraluminal concretions as well as ana tomic peculiarities can be reliably detected. Farther it is due to the simple structure of the Instru possible to manufacture this as a single-use instrument.

Further advantages result from the description and the attached drawing. Likewise, the above and the features listed further according to the invention each  because individually or in any combination with each other be used.
