Abstract

The invention relates to a pharynx side implant with a first and/or a second implant (1; 1'), each of which have two implant parts (46, 52; 60, 61), each with a front tongue side edge side or pharynx side wall side or soft palate edge side end (50, 56; 72, 74) and a back cervical vertebrae side end (48, 54; 76, 78). The two implant parts (46 and 52 and/or 60 and 61) of the implants (1, 1') are preferably bent in such a way that the convex side faces towards the inside of the pharynx or the pharyngeal mucosa when the implant is implanted. The back end (48, 54; 76, 78) of the implanted implant parts is fixed to or rests on the cervical spine (38) or rests on the tissue in front of the same or is implanted (fixed) in the tissue in front of the same.

Description

Die Erfindung betrifft ein Rachenseitenimplan tat.The invention relates to a pharyngeal side implant did.

With the obstructive sleep apnea syndrome, in fol as in the technical literature for short OSAS called, occur in the deep sleep phases and REM sleep phases (REM = Rapid Eye Movement) recurring Breathing breaks in which a muscular relaxation and as a result, a fall back of the Tongue body occurs due to gravity. The In extreme cases, the tongue body lies against the Ra rear wall and thereby closes the air away in this area. Furthermore, a col label the throat side walls to close the Respiratory tract.

The breathing breaks caused by this end only when the oxygen desaturation of the Blood and a lack of oxygen in the brain suddenly Waking up occurs until a flatter sleep level or full awakening is reached where with the muscular basic tension again so far takes that the tongue body from the pharynx retracted or the throat side walls tighten again and clear the airway. After that The sleep slowly flows back into a REM or deeper sleep phase until the above described  starts again.

The OSAS needs treatment because of those that recur regularly while sleeping Desaturation of blood in the myocardium coronary artery disease, cerebral vascular disease genes with an accumulation of stroke and small brain as well as high blood pressure crises and a variety of hormonal disorders can occur. Also leads the frequent absence of deep sleep, the disturbances REM sleep phases and frequent awakening Listlessness, daytime tiredness and easy on propensity to sleep during the day, what when operating machine or driving an accident can.

It is known that by creating an constant overpressure in the nasopharynx using an extracorporeal compressor during sleep with a hose and a nasal mask act. Such treatment with overpressure can permanent widening of the alveoli and Shortening of the walls of the alveoli, the so-called pulmonary emphysema, and there as a result from to a reduction in the total lung diffusion surface and shortness of breath. In addition, the Disadvantage that with a lung overpressure Pump capacity of the right ventricle must increase in order the same amount of blood against the increased back pressure to be able to press into the lungs. This can cause muscle lead to diseases of the right heart. Another Disadvantage is that wearing a mask is disturbing while sleeping.

Through the book by L. Grothe, H. Schneider "Sleep apnea and cardiovascular diseases", Thieme 1996, oral prostheses are known that before  to fall asleep in the throat and have the purpose of pushing the base of the tongue forward ken. However, such oral prostheses are only for slight degrees of OSAS promise success and also because of the complicated Handling and inconvenience from just one low percentage of patients tolerated.

Surgical are also used to treat OSAS Therapy measures known, which are essentially in an improvement in the passage of air in the nose and Nasopharynx area. It's even ver been searched, lower and upper jaw or one out saws the quadrilateral mandible in the chin area tongue muscles attached to it, so to improve the air passage. This is a big one surgical intervention, which is a considerable burden represents for the patient and moreover rarely gives acceptable results because, like the previously described surgical therapy measures, treated only indirectly because the falling back of the Tongue body and its application to the back of the throat wall and the associated obstruction of the airway is not causally avoided.

In the unpublished German patent application with the official file number 197 56 956 an implant for implantation in a tongue wrote that a fastener for fastening on the hyoid bone or on the side of the tongue NEN tissue and a support member, which differs from the fastening part in the area of the base of the tongue and essentially transverse to the direction of the tongue stretches.

The invention is based on the object Specify the pharyngeal implant through which the OSAS  is effectively avoided or reduced and leg impacts of the patient such as difficulty swallowing and largely avoided disturbances during sleep will, especially in the event that the Zun Basic implant according to the German patent application 197 56 956 are not sufficient to treat OSAS should.

The object underlying the invention will by the teaching specified in claim 1 ge solves.

The basic idea of this teaching is to be a To create the pharyngeal side implant that so implanted bar is that one end of the implant neck vertebral side and the other end of the side of the tongue is arranged. The revenge egg according to the invention Tenimplantat can be used advantageously if the supportive action of the tongue base mentioned above plantates are not sufficient and a basic tongue implant reinforcement of the deed or a change of basic tongue implant for various reasons should. A possible reason for this would be that one too great impairment of basic tongue mobility is feared or that the base tongue implant so is well healed that another operative mani excessive scars in the implant area education or otherwise worsening of Results of the operation. The main we Kung the pharyngeal side implant according to the invention in this case would be the support of the edge of the tongue; a side effect would be the support of the side Throat tissue. The pharynx in the invention plantat can be used advantageously even if another major cause of OSAS in the Collapse of the lateral pharynx walls is seen.

The main effect of the pharyngeal side implant would be in in this case the support of the lateral pharynx bes, whereby as a side effect the support of the Tongue edge is added. By the invention The side of the tongue can be implanted with the throat be supported more than when used alone of a base tongue implant without the entire Transfer support force across the base of the tongue is, despite a stronger lateral increase support remains more flexible than with sole Tongue base implant and swallowing less is affected because the supporting force in ge whole throat cross-section and / or over part of the Longitudinal expansion of the pharynx better distributed.

The implant parts of the Implan are advantageous tate curved, such that in the implant the convex side of the inside of the throat or facing the pharynx mucosa. Hereby are particularly good support effects on the seitli Chen tongue edge and / or the lateral pharynx tissue exercised.

Further advantageous and expedient training gene of the task solution according to the invention are in the further subclaims specified.

The invention is based on the following attached drawing that shows exemplary embodiments are explained in more detail.

Fig. 1 shows the arrangement of a first In plantats 1 with a left implant portion 46 and ei nem right implant member 52. The implant parts 46 , 52 each have a front end 50 , 56 on the edge of the tongue and a rear end 48 , 54 on the side of the cervical spine. The implant 48 , 54 on the cervical vertebra side can be connected to the spine 38, for example by means of a screw; however, it can also be arranged at a distance from the spine and end in the tissue adjacent to the spine, as a result of which a greater or lesser degree of mobility is maintained in all directions for this implant end, the mobility in the direction of the spine being limited by the latter itself. The mobility in the other directions is mainly limited by the elasticity of the tissue located between the end of the implant and the spine, but also by restoring forces of the rest of the tissue adjacent to the implant. The front end 50 , 56 of the implant parts on the side of the tongue side can be arranged in the region of the edge of the tongue side or rest against the side of the tongue side or be implanted in the side of the tongue side. The two implant parts 46 , 52 are preferably planted through a slot in the lateral tongue back 4 and each extend behind the Gau menmandel in a curved shape up to the cervical vertebra by 38 . The bent implant parts are arranged so that in the implanted state the convex side of the implant parts faces the inside of the throat or the pharynx mucosa. Approximately the middle of the implant parts 46 , 52 acts as a supporting part for the side wall of the throat and prevents its collapse towards the middle of the throat. The implant parts 46 , 52 of the implant 1 can thus perform two functions, namely supporting the lateral tongue areas against falling back and supporting the side wall of the throat against collapse directed towards the middle of the throat.

Fig. 2, which shows the oral cavity from the front with the palate and cheek mucosa removed on the left, shows on the one hand the arrangement of the implant 1 described in connection with FIG. 1 and the arrangement of a further implant 1 '. With regard to the arrangement and design of the implant 1 , reference can be made to the description of FIG. 1.

The further pharyngeal side implant 1 'also has two implant parts, a left implant part 60 and a right implant part 61 . These in the implant parts 60 , 61 are preferably planted at an angle of approximately 45 ° to the implant parts 46 , 52 and serve in particular to support the side wall of the throat. These implant parts 60 , 61 ver run obliquely from the top through the front lateral Gau mensegel backwards down in front of the lateral spine and have no contact with the tongue. They each have a front end of the soft palate or throat wall 72 , 74 and a rear end of the cervical vertebra 76 , 78 and, like the implant parts 46 , 52 of the implant 1 , are designed so that the convex side of the inside of the throat or the pharyngeal mucosa is facing. The use of the implant 1 ', that is to say the implant parts 60 and 61 , has particular advantages if the throat above the back of the tongue, that is to say in the region of the palate arches, collapses in sleep and this region is to be supported. The rear end 76 , 78 of the implant parts 60 , 61 of the implant 1 'is fastened to the cervical spine 38 or is supported on it or is fastened in the tissue in front of it or is supported on it. The front end 72 , 74 of the inserted in the implant parts 60 , 61 is fastened in, or supported on, the tissue located in the vicinity of the hard posterior edge 6 of the palate and / or the pterygoid bone 70 .

Fig. 3 shows the right implant parts 52 of the implant 1 and 61 of the implant 1 'according to the invention in the respective implanted position, the right side face with opening of the right mouth and right nose being removed in the drawing for better illustration is.

The implant parts 46 , 52 , 60 , 61 are essentially strip-shaped, whereby strip-shaped should also be understood to mean a flat but, at different locations, different thicknesses and different widths. The longitudinal extent of the implant parts is preferably greater than the implant thickness, and the transverse extent can be variable. The implant parts have, at least after the implantation, a shape which is convexly curved with respect to the side wall of the pharynx.

The implants 1 and 1 'can be used individually and in combination with one another or individually or together in combination with a basic tongue implant, in particular if, from the outset, essential reasons for the OSAS are both the falling back of the tongue and the collapse of the pharynx is proven. The choice of implants depends on which structures are essential in collapsing sleep.

The lateral pharyngeal implant according to the invention te are preferably very soft to the essential structures such as nerves and vessels are not to irritate or hurt. They consist of bred tetem, the body's own tissue or plastic or a combination of both materials. Instead of ge cultured, endogenous tissue can also change the body's own tissue taken from the site can be used.
