Description

The invention relates to molded parts, in particular films, to promote the new formation of bone material in the jaw. she relates in particular to a subpereosteal framework for the new bone formation in animal and especially human Chen pine, and especially here in the alveolar ridge.

As is well known, the problem that teeth from the Jaws have to be operated out and valuable kno substance is lost. In the empty alveoli, as in extraction of a tooth usually sets a blood cake from which, after shrinking in the course of Time forms bone material. However, several were next to each other If the teeth are operated out, you have previously had the Alveo Lark combs by inserting non-absorbable or resor foil or metal mesh, especially titanium mesh shielded and this after new formation of the bone substance in a second operation. The resorbier edible foils, however, had the disadvantage that they were insufficient had adequate dimensional stability. The use of absorbable Foils, on the other hand, had the advantage that they had no additional surgery, like the others, required Methods including autologous knot transplantation material or when bone replacement material is inserted is required. With this there was also the risk that there were repulsions.

It is therefore known in principle (cf. DE 42 26 465 A1), with With the help of molded parts made of bioabsorbable polymers, e.g. B. Polylactic acids, jaws (bones) and roots (operationally) have been removed or missing, restore and tooth sausage to attach the tent. The material can e.g. B. U-shaped, tube-like Lich or horseshoe-shaped. The restoration of the Kie bone can be accomplished by looking at the shaped one Material according to the shape of the broken area of the jawbone suitably cuts, autogenous particulate spongy knot filling material and marrow into the reproducing material, the reproducing material in the form of a defect a U or an inverted U in three-dimensional configuration ration covered, followed by the binding of the reproductive Material with threads on both sides of the bone on one de perfect place to restore the material To fix the jawbone.

This fixation is generally carried out with a foreign Ma materials such as metal wire or polyethylene screws, however, Example 4 also describes instead of the Me tall wire a thread made of poly-L-lactic acid, i.e. a bio use absorbable material. On the other hand, he will believes that the invention there "includes a fixing device for artificial roots slabs placed on the jawbone Reproduction material are fitted exactly ". The writing Wise "roots plates" here means that the fixation device should consist of plates for artificial roots. How to from the further disclosure and the figures, especially 1 and 3, results, the term "plate" is used here for a network or Tissue or reticular shell used.

In US Pat. No. 5,700,479 molded parts made of two foils are wrote, at least one of which is perforated. You should also have spacers such as knobs that the should keep both foils in a parallel position. The element may be biodegradable, i.e. i. in resorbierba from the body rer form, and the two films can vary  have stiffness. These elements are fixed with the tooth connected.

Formteile, die als auf dem Kiefer hohlraumbildende, gut fixierbare Folien oder Platten vorliegen und so gestaltet sind, daß eine feste Verbindung mit der Knochenunterlage auf dem Kie fer ermöglicht wird, sind in dieser Entgegenhaltung weder be schrieben noch lassen sie sich daraus herleiten.DE 693 08 561 T2 = EP 649 300 M1 describes the use of a Ge mix of two amorphous polylactic acids to produce egg described an implantable bioresorbable membrane, "the to promote the regeneration of the gums and / or the Jaw bones using the technique of controlled tissue rains for the treatment of periodontal diseases are. Such membranes are not dimensionally stable, however from the information shows that the membrane has the desired softness degree at a temperature of no more than 37 ° C should. Moldings, which as cavity-forming on the jaw, good fixable foils or plates are available and are designed that a firm connection with the bone pad on the Kie fer is not permitted in this document wrote nor can they be derived from it.

Der Gegen stand der vorliegenden Erfindung ist aus dieser Entgegenhaltung weder bekannt noch daraus herzuleiten.DE 196 11 285 A1 describes protective plates or foils wrote that made of biocompatible thermoplastic exist, such as the poly (L-lactide-co-D, L-lactide). The protection plate should serve to create temporary protection with which fixes the graft in place and against it the bony graft bed is to be pressed. The opposite State of the present invention is from this citation neither known nor derived from it.

In DE 195 20 867 A1 there is a covering membrane made of body compa tiblematerial for temporarily covering one with bones building material, especially hydroxyapatite granules, filled Recess described in the body's own bone tissue. Since the Membranes only have insufficient dimensional stability a retention support device to the cover membrane ordered the at least one mechanical retention aid points. For the same reasons, DE 195 43 906 A1 provided membranes for controlled bone regeneration to be provided with a support structure that does not fit into the membrane is only on the surface of the kno  Chenreegeneration held room and after the New bone formation must be removed again. Through the support The scaffold will cause the membrane to lose its stiffness is missing, is kept in the desired shape.

These disadvantages are avoided by the invention. Against State of the invention are molded parts, especially stiffened fo lien, to promote the new formation of bone material in the jaw fer that is biocompatible, deformable, can be cut and integrated set condition, stable and above all resorbable are cash. The use of such molded parts makes it possible to to go completely new therapeutic paths in dentistry.

The invention relates to molded parts which are suitable for enabling or promoting the new formation of bone material in the jaw, which are biocompatible, deformable under pressure and / or heat, under the conditions in the jaw and tooth region for the duration of the new formation of bone substance are stable and dimensionally stable, in a form that can be absorbed by the body as cavity-forming, easily fixable spatial shape on the jaw and are designed so that a firm connection with the bone support on the jaw is made possible, characterized in that they are present as foils or plates and

  • a) über eine Vorfalzung in einer für den neuen Kieferkammauf bau geeigneten Weise verfügen odera) via a pre-fold in one for the new alveolar ridge suitable construction or
  • b) so gestaltet sind, daß zwei an ihren interdentalen Enden unterschiedlich geformte Folienteile stabil verbindbar sind oderb) are designed so that two at their interdental ends differently shaped film parts can be stably connected are or
  • c) die Form einer apicalen Verbreiterungsfolie haben.c) have the shape of an apical widening film.

The concept according to DE 42 26 465 A1 is diametrically the Invention opposite, according to which the molded parts firmly on the Bone pad to be attached. Because that through The problem solved according to the invention is that Bone formation on the existing alveolar bone thereby promote that the molded parts to the shape of the spatial structure in which the bone substance is to be regenerated, adapted  are. Depending on the possibility and necessity, these are then included absorbable surgical screws or rivets, in particular made of the same material, firmly on the bone base attached. So it becomes possible that alveolar ridges even after long previous extractions or traumas rebuilt who the.

The dimensional stability of the moldings according to the invention causes white ter that the wound resulting from tooth extraction is stable is shielded from the action of mechanical forces, so that the blood cake under the protection of the molded part, e.g. B. of Can reorganize frame or box to bone. A sol che conception and such possibilities are the DE 42 26 465 A1 not to be removed. The fact that according to example 1 new bone material in the region between the tooth and the Bone is formed at the places where the Element was arranged, means a different situation than the formation of bone material on atrophied alveolarkno Chen or additionally forming alveolar bones for ver widening of an apical base.

Possible materials for the molded parts are which has the aforementioned properties and also one have adequate dimensional stability, d. H. a space-giving support radio exercise before the material is absorbed. This shape Stability should be used to treat large defects in general three to six months, preferably at least four or even five months. On the other hand, it can also be sufficient be pro for the treatment of small extraction sockets to use products whose dimensional stability for a shorter time, e.g. B. is maintained for two to six weeks.

Absorbable substances suitable for the invention are, for example, the polydioxanone or self-reinforced polyglycol lid and in particular the polylactide of the formula (C 3 H 4 O 2 ) n . Of these, the copolymer poly (L-lactide-co-D, L-lactide) in a molar ratio of L-lactide to D, L-lactide of 67:33 to 73:27 is very particularly preferred, and of these in turn the commercially available poly ( L-lactid-co-D, L-lactid) 70: 30, which is available under the brand name Resomer LR 708 from Boehringer Ingelheim. While pure poly (L-lactide) has a very long breakdown time up to five or more years for complete absorption, the copolymerization of L-lactide and D, L-lactide reduces the complete breakdown time to about three years without affecting the mechanical properties compared to pure poly (L-lactide).

The molecular weight, and thus the degradation time and shape stability, ie the maintenance of the spatial support function of the materials used according to the invention, and in particular of the polylactide, can vary within wide ranges by modification. What is important is the fact that the materials in the body metabolize to CO 2 and H 2 O, that is, they are biocompatible, non-toxic and do not lead to tissue defense reactions. The lower the molecular weight, the faster the substance is absorbed and the shorter the shape stability. For example, the aforementioned lactide 70:30 has a molecular weight of approximately 160,000 to 600,000, depending on the pretreatment.

The polylactide is preferably in an amorphous form, the is free or practically free of crystalline components. The fully amorphous forms are for practical use most suitable.

The use of the aforementioned substances - the Resomer LR 708 is e.g. B. as white or almost white granules in the trade - for Manufacture of molded parts, e.g. B. by injection molding, enables in completely new therapeutic approaches in dentistry. Because the mate rialien with tempera moderately above the body temperature doors, e.g. B. above 45 ° C or 50 ° C or 55 ° C. can, the molded parts to the shape of the spatial structure, in to which the bone substance is to be regenerated be fitted. The molded parts preferably have the shape of Fo lien or panels that are imperforate or for the better Perforated mass transport for feeding the new tissue  be put. These can be used to create three-dimensional, molded parts supported by knobs, blocks or ribs with the The subperiosteal part of the nub is introduced to the bone and depending on Possible and necessary with absorbable surgical Screws or rivets, especially of the same material, be firmly attached to the bone pad. It will be possible Lich that alveolar ridges, even after a long time ago Extrak ions or traumas are rebuilt to a static and to achieve aesthetically better prosthetic restoration. So can be used especially for implant restorations in the upper and in the Un better conditions are created.

Eine besonders geeignete Ausführungsform hierfür sind soge nannte Platzhalterformkörper (bone space frames).A particularly suitable embodiment for this are so-called called placeholder shaped bodies (bone space frames).

The mode of action can be explained well on fresh wounds. A blood cake is initially formed there. As the wound through the moldings according to the invention stable from the action me chanic forces are shielded, this blood cake can under the protection of the frame or box from the fiction, ge Reorganize into bone according to the material used. If that This process is finished, which is also due to the nature of the patient depends, the molded part according to the invention can be without damage Lose support function and be reabsorbed.

A two-piece, preformed, in mutually stable application form for vertical, hori Central and combined periodontal defects ideal. This can ve stibular and lingual in the manner previously indicated on the in terradicular bone base. If you approxi once snapped together, it forms a stable one interdental pad for the ver sliding lobe and also serves as a space generator for bones new formation in the periodontal area. In addition, by the two-part application form the tedious interdental through saves threading.

As shown in Fig. 3, a widening of the apical base can be used in a vestibularly subperiosteal form, tapering towards a cervical garland, the semi-radii of which are shaped individually, ie depending on the area of application, for anterior teeth, premolars or molars. The z from the bottom. B. up to 2 mm high spacing knobs or ribs can be individually shortened and should be coordinated from a distance to the respective interradicular situation, in order to allow safe treatment in addition to the prophylactic use especially for the orthodontist. The preformed papilla girls can be individually pre-operative adjusted on a sterile model and thereby enable fast operative care. For the surgical procedure, it is suggested that after a vertical, interradicular mucosal incision between the canine tooth and the first premolar, the periosteum must be loosened to the required width and, after careful horizontal tunneling, the respective shaped bodies inserted horizontally and closed at the ends with the screws specified above on the bone base fix.

Darin bedeuten die Maßangaben mm.The invention is he through the accompanying figures purifies. The dimensions mean mm.

In Fig. 1 a universally applicable film is shown in the dimensions 30 × 40 mm, in Fig. 1a in the top view, in Fig. 1b in the side view of the long side, in Fig. 1c in the narrow side view and in Fig. 1d in a way that is suitable for a new alveolar ridge construction. 1 and 2 represent knobs which are not to scale, the arrangement, thickness, length and number of which are variable. 1 denotes the vertical and 2 slightly shorter horizontally supporting knobs. In Fig. 1d it is shown that the right part has already tilted up, which is also indicated by the continuous dashed line, while the left part is still at an angle of 45 °. The kinking is facilitated by pre-folding the film. The arrangement shown here and the dimensions are only examples. Naturally, the shape of the film depends on the local conditions in the jaw; the film can be round, oval or have any other required shape. In Fig. 1a, a 7 mm wide alveolar ridge surface is drawn. The side parts next to it are then placed on the outside and inside, i.e. on the vestibular and lingual bone surface, and fixed with screws or pins made of the same material as the molded part.

Fig. 2b und 2c stellen die Seitenansichten dar. Fig. 2 shows the two-part application for periodontal defects. Fig. 2a shows the two film parts 3 and 4 with a length of about 15 mm in a size customary in practice. FIGS. 2b and 2c represent the side views. Figs. 2d and 2e show a possible use of this film, wherein the hatched portion 5 shows the originally missing and build the bone part.

Fig. 2a und 2c). The two film parts are shaped differently at their interdental ends 6 and 7 , as a result of which they are stably connected and pushed into one another and are easy to handle for the attending doctor (see FIGS. 2a and 2c). The fixed fi xing takes place at point 8 in Fig. 2e and is necessary both vestibularly and orally.

FIG. 3 shows a view of an apical widening film for a lower jaw front, FIG. 3a showing a top view and FIG. 3b the side view of the long side and FIG. 3c the transverse side. The knobs 1 can again be varied in number and position and in terms of thickness, whereby they are supported horizontally against the base. This shows the special pre-shaping of the individual gingiva course corresponding to the anterior or posterior region to broaden the apical base.
