The suture of the invention for surgical wound closure, especially in the area of soft tissue, has a one-piece suture body. According to the invention, the cross-sectional area of the suture body has a plurality of irregularly shaped radial indentations, the indentations extending substantially over a majority of the axial length of the suture body.
The large number of axially extending indentations results in a high surface area in relation to the volume, so that the suture yarn can distribute forces that can occur, in particular when suturing wound sites, over a larger area. At the same time, the material of the sewing yarn can yield elastically substantially over the entire axial length in the radial direction, so that stress peaks and locally increased shear forces can be automatically reduced. This reduces the risk of cutting the suture into the tissue as well as necrosis of the tissue, which can be caused by over-compression of the tissue, thereby reducing blood supply to the tissue. Especially with soft tissue, which can withstand significantly lower shear and cutting forces compared to the other tissue of a human body, an impairment of the wound healing process is reduced. At the same time, it should be taken into account that a certain prestressing of the sewing thread on the fabric is required in order to keep a wound site securely closed even after a few flow phenomena of the tissue cells. Further, the increased surface may provide a greater frictional force after knotting the suture yarn so that a knot made with the suture yarn will hold better and, if desired, a simple knot will suffice instead of a double knot. Moreover, the one-piece suture can be easily produced by extrusion without intertwining a plurality of single fibers, thereby reducing the manufacturing cost. The monofilament suture yarn thus consists in particular of a single fiber, and depending on the application, it may be advantageous to join two or more integral suture yarns into a multifilament thicker sewing yarn, for example by twisting them into a thread. Preferably, the suture consists of a single staple fiber or filament. Due to the automatic reduction of stress peaks, a comparatively thin suture yarn can be used, so that costs for wound care can be further reduced. Such a suture yarn can be used in particular for textile implants, whereby the migration of the implanted foreign body can be reduced.
The suture body is in particular made of a biocompatible material, in particular plastic material, as already used for known surgical sutures. The suture may be made of a non-resorbable material such as polyvinylidene fluoride (PVDF), polypropylene (PP), polyester (PE), polyamide (PA). Preferably, the suture body is made of a resorbable material, for example polylactide (PLA), polyglycolide (PGA), copolymer polylactide polyglycolide (PLAcoPGA), Polycaprolactone (PCL), Copolymer Polylactide Polycaprolactone (PLAcoPCL), Copolymer Polyglycolide Polycaprolactone (PGAcoPGA). The indentations are configured, in particular, as grooves extending essentially continuously in the axial direction, wherein interruptions in the axial course can occur due to statistical fluctuations in the production process, or two indentations can collapse in a common indentation in a secondary manner. The notches may be substantially V-shaped, wherein due to the elasticity of the suture the respective notch may be compressed, stretched and / or deformed. In particular, undercuts may result in the notches in the radial direction.
The geometry of the cross-sectional area of the suture can be suitably produced in particular by extrusion, whereby a comparatively irregular suitable shape of the cross-sectional area can result, which can only be described in an insufficient and incomplete geometric manner. For example, the cross-sectional area may substantially have the contour of a snowflake, whereby a variety of snowflake shapes is possible. In particular, can be produced by the manufacturing parameters during extrusion and the geometry of nozzle openings of a die used a variety of different suitable geometries for the cross-sectional area of the suture, so that the specific geometry of the cross-sectional area of the suture is very much due to the manufacturing process and defined by this and described can be.
In particular, the indentations are distributed in the circumferential direction and at least two indentations have an extension direction in different planes. The planes of the direction of the notches, which extend in different radial directions, can be used to provide a local compliance over the circumference of the seam yarn. This allows forces to be distributed substantially uniformly over the circumference of the suture yarn. At the same time a preferred direction of the suture yarn can be avoided, whereby the suture can be easily sewn and linked.
Preferably, several notches have a different notch depth. Due to the different notch depths different elastic spring effects can be adjusted, which facilitate an automatic alignment of the suture yarn to the tissue at an attacking force.
Particularly preferably, the suture body has a surface A for which 1.2 ≦ A / A r ≦ 3.0, in particular 1.3 ≦ A / A r ≦ 2, compared to a surface A r of a cylindrical suture body with the same size cross-sectional area , 5, preferably 1.4 ≦ A / A r ≦ 2.0, and more preferably A / A r = 1.7 ± 0.2. The exact ratio A / A r can change in the axial direction due to static fluctuations in the manufacturing process and does not necessarily remain constant. In comparison to a circular cross-sectional area of the suture yarn results in a much larger surface that can absorb forces occurring with lower voltage spikes.
In a preferred embodiment, the cross-sectional area of the suture body has an area moment of inertia I for which 1.10 ≦ I / I r ≦ 2.00, in particular 1.15 ≦, compared to an area moment of inertia I r of the cross-sectional area of a cylindrical suture of equal size I / I r ≦ 1.70, preferably 1.18 ≦ I / I r ≦ 1.50, and more preferably I / I r = 1.25 ± 0.05. The exact ratio I / I r can change in the axial direction due to static fluctuations in the manufacturing process and does not necessarily remain constant. This results in comparison to a seam yarn with a circular cross-sectional area a larger radius of curvature and thus a more uniform load on the tissue.
In a particularly preferred embodiment, the cross-sectional area of the suture body has a circular core without indentations and an ideal circular envelope surrounding the cross-sectional area radially outside, wherein the core has a diameter D i and the envelope has a diameter D a for which 1.1 ≦ D a / D i ≦ 4.0, in particular 1.3 ≦ D a / D i ≦ 3.0, preferably 1.5 ≦ D a / D i ≦ 2.5, and more preferably D a / D i = 2.0 ± 0.2 applies. The exact ratio D a / D i may vary in the axial direction by static fluctuations in the manufacturing process and need not necessarily remain constant. Further, the circular core and the envelope do not necessarily have to be concentric with each other. The core, into which the indentations do not protrude, ensures a minimum basic stability, whereby a resilient yielding of the material of the suture yarn can be allowed between the core and the envelope, in order to better distribute occurring forces over the circumference.
The invention further relates to an extruder die for extruding a stitching yarn, wherein the stitching yarn may be formed and developed as described above. The extruder die according to the invention has a plate body which has at least one nozzle opening for extruding exactly one seam yarn, wherein the nozzle opening has at its exit in the direction of extrusion a plurality of substantially radially extending to a center slots having a slot width b and the slots at the center-facing end have a minimum distance s i to one another for which 0.01 ≦ s i / b ≦ 0.20, in particular 0.02 ≦ s i / b ≦ 0.15, preferably 0.03 ≦ s i / b ≦ 0.10 , more preferably 0.04 ≦ s i / b ≦ 0.07, and more preferably s i / b = 0.05 ± 0.01. The individual slots are arranged so close to one another that the material extruded through various slots of the extruder die can collide in the direction of extrusion behind the extruder die and form a one-piece seam body. At the same time, by the material of the extruder die between the slots, radial indentations may be created in the seam yarn which may extend substantially the entire axial length of the extruded seam yarn. The large number of axially extending indentations results in a high surface area in relation to the volume, so that the suture yarn can distribute forces that can occur, in particular when suturing wound sites, over a larger area. As a result, an inexpensively produced seam yarn is created, which reduces impairments of a wound healing process.
The slots of the plate body in particular have a constant width, wherein it is also possible that, for example, increases the width radially outward. In particular, the extruder nozzle has a multiplicity of nozzle openings which, for example, can be arranged at different distances from one another in different rows, with adjacent extruder nozzle pairs preferably having a substantially equal spacing. The extruder die can be provided in an extruder, in particular at an exit of the extruder. The extruder may preferably be heated and set at an adjustable pre-screening speed to a suitable pressure at the extruder die.
In particular, at least one slot branches radially outwardly in a substantially Y-shape into at least two partial slots, the partial slots in particular symmetrical to a center radial direction include an opening angle α, for the 40 ° ≤ α ≤ 100 °, in particular 50 ° ≤ α ≤ 90 °, preferably 60 ° ≤ α ≤ 80 °, and more preferably α = 70 ° ± 5 ° applies. This makes it possible to set a notch with a smaller notch depth in the region of the Y-shaped branch than between two slots extending essentially to the center. Furthermore, in comparison to a purely star-shaped cross-sectional area of the suture in the radially outer region, material can be provided for the suture, whereby the surface of the suture can be increased and a larger contact surface can be provided on the tissue at the outer circumference of the suture.
Preferably, the nozzle opening, starting at its inlet in the direction of extrusion, has an inlet channel communicating with the slots with an average diameter d k for which 0.10 ≦ b / d k ≦ 0.80, in particular 0.15 ≦ b / d k ≦ 0 , 60, preferably 0.20 ≦ b / d k ≦ 0.40, and more preferably b / d k = 0.3 ± 0.05. The material to be extruded can thus be pressed first into the inlet channel and from there to be pressed through the slots, for example, through a region of the nozzle opening which is designed like a template. As a result, the material to be extruded can be pre-compressed in the inlet channel, whereby a sufficiently high pressure can be built up in the nozzle opening so that the parts pressed through different slots can grow together behind the nozzle opening to form a common one-piece sewing thread.
The invention further relates to a method for producing a suture for surgical wound closure, which may in particular be formed and further developed as described above, wherein a curable material through an extruder die having at least one nozzle opening having a plurality of substantially radially extending to a center slots is pressed, in particular an extruder die is used, which may be as described above and further developed. The individual slots may be arranged so close together that the extruded through different slots of the extruder die material can collide in the extrusion behind the extruder die and forms a one-piece weld body. At the same time, radial notches can be created between the slots through the material of the extruder die which can extend substantially the entire axial length of the extruded suture yarn. The large number of axially extending indentations results in a high surface area in relation to the volume, so that the suture yarn can distribute forces that can occur, in particular when suturing wound sites, over a larger area. As a result, an inexpensively produced seam yarn is created, which reduces impairments of a wound healing process.
Preferably, a plastic material, in particular PVDF, at a temperature T of 200 ° C ≤ T ≤ 280 ° C, in particular 220 ° C ≤ T ≤ 260 ° C and preferably 235 ° C ≤ T ≤ 250 ° C and a pressure p of 40 bar ≤ p ≤ 80 bar, in particular 50 bar ≤ p ≤ 70 bar and in particular p = 60 ° bar ± 5 bar, in particular with a speed v of 550 m / min ≤ v ≤ 800 m / min, preferably 600 m / min ≤ v ≤ 750 m / min, more preferably 650 m / min ≤ v ≤ 700 m / min, and most preferably 685 m / min ≤ v ≤ 695 m / min, pressed through the extruder die. These process parameters are particularly suitable for air-cooled suture yarns, the cross-sectional area of the air-cooled suture yarns in particular having an average diameter d of 1 μm ≦ d ≤ 350 μm, preferably 10 μm ≤ d ≤ 100 μm and particularly preferably 25 μm ≤ d ≤ 30 μm. This results in a substantially schneeflockenfömige cross-sectional area of the seam yarn. When using other base materials and / or changes in dimensioning, for example, the nominal diameter of the sewing yarn, the process parameters can be adjusted accordingly.
Preferably, after pressing the curable material through the extruder die before and / or after an optionally provided cooling, stretching and / or thermofixing of the seam yarn takes place. The suture can thereby be made more stable and more dimensionally stable.
The invention further relates to a suture for surgical wound closure, in particular in the area of soft tissue, which can be produced by the method described above. The sewing yarn is in particular as described above and further developed. The large number of axially extending indentations results in a high surface area in relation to the volume, so that the suture yarn can distribute forces that can occur, in particular when suturing wound sites, over a larger area. As a result, an inexpensively produced seam yarn is created, which reduces impairments of a wound healing process.

In a comparative experiment, the human tissue was simulated using the substitute medium "gelatin". The mechanical properties of human tissue are primarily determined by the structural protein collagen. By partial hydrolysis, the collagen can be brought into a soluble form, the gelatin. The test setup provides to guide the suture to be tested by means of a needle through a defined gelatin block. One end of the gelatin block is attached to a lower clamp of a tensile tester while the suture ends of the suture to be tested are secured to an upper clamp of the tensile tester. The respective seam yarn is subjected to a test force of 0.5 N and then the force drop is measured over a defined time window. The application of the test force, in the real application case, essentially corresponds to the knotting of the sewing thread and the measured force drop corresponds to the cutting of the sewing thread into the fabric.
