技术领域
本发明涉及一种晶状体递送系统。
背景技术
诸如人工晶状体(“IOL”)的眼内植入物可通过在角膜中形成的小切口递送到眼睛中。已经研发出递送装置来辅助将这种植入物递送和插入到眼睛中。
角膜或巩膜切口允许通入眼睛,并且切口越小,造成的损害越小,所需的切口愈合时间越短。此外,优选的是,在递送过程中人工晶状体不受到损坏,或者至多受到不会影响该人工晶状体的功能的最小程度的损坏。
取决于人工晶状体的物理特征(例如形状、尺寸等),在递送过程中人工晶状体的形状和/或构造需要被改变或者减小尺寸,以使得人工晶状体能够通过小切口插入。晶状体的构造/形状的尺寸减小或者调整允许更小的递送外形轮廓。
因此所需的递送装置应能减小人工晶状体的递送轮廓,使其能够通过小的切口递送入眼内。此外,递送装置应最小化并且优选的是消除在递送过程(包括将人工晶状体装载到递送装置内)中对晶状体的损坏。
发明内容
本发明的一个方面涉及一种将人工晶状体以液压方式装载到递送系统中的方法。该方法包括将人工晶状体定位在压缩室内并邻近递送装置,其中压缩室和递送装置流体连通。该方法包括使流体流过压缩室并进入递送装置,其中使流体流过压缩室包括将人工晶状体装载到递送装置中。

具体实施方式
本发明总体涉及用于将眼内植入物、诸如人工晶状体(IOL)递送穿过眼睛中的切口的递送装置。递送装置通常对人工晶状体(或至少部分人工晶状体)进行压缩并增大其长度,使其进入递送构型,从而使得人工晶状体能够被递送通过相对于人工晶状体的尺寸而言小的切口而进入眼睛内。此外,递送装置在递送过程中使得对人工晶状体的剪切力和张力最小化,以使得对人工晶状体的破坏最小化,优选没有破坏。
此处描述的人工晶状体为在天然晶状体已被从眼睛移除之后被植入在晶状体囊内的可调节型人工晶状体。特别是,人工晶状体包含诸如流体的可流动的介质,其能够响应于睫状肌的运动而在人工晶状体内运动以改变人工晶状体的放大率。这样的示例性人工晶状体在如下文献中有更充分地说明:2002年12月12日提交的美国临时申请No.60/433046;美国专利No.7122053、No.7261737、No.7247168和No.7217288;2006年12月19日提交的美国专利申请No.11/642388;以及2006年12月27日提交的美国专利申请No.11/646913,这些文献的全部公开内容在此通过引用结合入本文。但是还应想到的是,此处描述的递送装置可用于递送其它类型的可调节型人工晶状体(例如,非流体驱动可调节型人工晶状体)、不可调节型人工晶状体,甚至其它类型的眼内植入物。此外,可以想到,递送装置可被用于将人工晶状体或其它眼科装置递送到眼睛的除了晶状体囊外的部分,诸如前房,或者在晶状体囊已被移除之后递送到后房。
递送装置通过将人工晶状体或者部分人工晶状体从扩展构型压缩到递送构型来减小人工晶状体的递送轮廓。在一些实施方式中,人工晶状体在被装载到递送装置之前呈大致圆形,但是可被递送装置压缩成加长的大致圆柱形。递送装置的一个优点是在递送过程中(包括装载和展开)这些递送装置使得作用于人工晶状体上的力的大小和/或类型最小化,这在递送过程中可有助于使人工晶状体的破坏程度最小化。对于易损的人工晶状体(例如由聚合材料(polyermicmaterials)构成)和/或包括多个互连组成元件的人工晶状体(在装载和展开过程中该人工晶状体的配合或粘合元件可能会由于作用于人工晶状体上的某些类型的力而被破坏)来说,这是有利的。
在优选的实施方式中,递送装置使得在递送过程中人工晶状体上的剪切力和张力最小化,代替使人工晶状体在压缩力下整形。
图1A-1C示出了示例性的流体驱动可调节型人工晶状体210,其可使用此处描述的递送装置被递送到晶状体囊中。人工晶状体210包括不可任选的周边部分,该周边部分包括接触部212和214。人工晶状体10还包括可选部分,该可选部分包括前部元件216、中间层218、后部元件或基片222。中间层218包括致动器220。接触部212和214限定内部容积224,该内部容积224与由后部元件222和中间层218限定的有效通道226流体连通。接触部与囊袋接合,使得小带松弛和张紧导致接触部的变形,这使得设置在接触部和有效通道内的流体在接触部和有效通道之间分配。当流体从接触部引向有效通道时,在有效通道中压力的增加使致动器220前向偏转,导致前部元件216的弯曲变得更急剧。这增大人工晶状体的放大率。该过程在任何上述列举的示例性专利申请和专利文献中有更详细的描述。
图2示出了递送装置2和人工晶状体100的示例性实施方式。人工晶状体100包括光学部分102和接触部104(见图3),它们以未受压或扩展构型定位在递送装置中。递送装置2包括插入器主体4、牵引滑块6和带8。牵引滑块被连接到带并位于插入器主体(底部未示出)的外表面和底面上的带部分上,而没有与插入器主体的内侧上的带相连接。牵引滑块沿箭头D或P方向的运动使位于插入器主体的内侧上的带部分沿着与牵引滑块的运动方向大致相反的方向运动。例如,当牵引滑块6沿着近侧或P方向运动时,位于主体内部的带部分大致沿着D方向运动。在牵引滑块被致动时,带大致用作传送带,以移动人工晶状体。
在使用中,当沿着近侧方向(图2中箭头P的方向)拉动牵引滑块时,导致位于插入器主体的内侧的带部分沿着插入器主体的内表面沿大致远侧方向(箭头D的方向)运动。位于插入器主体的外侧和底部的带部分也沿着近侧方向运动。随着位于插入器主体的内侧的带部分向远侧运动,它们最终绕着插入器主体的远端5运动到插入器主体的外侧。
类似的,当沿着远侧方向或箭头D的方向推动牵引滑块时,位于插入器主体的外侧和底部的带部分沿着远侧方向运动,并且位于插入器主体的内侧的带部分向近侧运动。这导致位于插入器主体内的人工晶状体沿着近侧方向移动。
递送装置被构造成仅仅带而不是插入器主体(或者插入器主体尽可能不)接合人工晶状体。因为人工晶状体不与插入器主体(或可增加的递送装置的任何其它部分)接触,在人工晶状体由带移动时,插入器主体不会在人工晶状体上施加张力或剪切力/应力。此外,因为带与人工晶状体一起运动,由带施加到人工晶状体的剪切力和张力的大小被最小化。如图2所示,在插入器主体的底面中形成有开口或空间10。形成插入器主体中的开口是为了避免插入器主体和人工晶状体之间的接触,以帮助使得人工晶状体上的不期望的力最小化。
为了将人工晶状体递送入眼睛中,人工晶状体被置于插入器主体的内部,基本上仅与带接触。人工晶状体被以扩展构型放置,使得其仅仅刚好与带接触(如图2所示)。沿着近侧方向致动牵引滑块,人工晶状体沿着远侧方向朝着装置的远端5移动。由于装置的远端与其近端7相比具有减小的宽度,随着人工晶状体向远侧移动该人工晶状体受到压缩并且之后从远端5排出。人工晶状体被从装置的远端递送入眼睛中,在从递送装置被释放之后人工晶状体在眼睛中扩展。
当在传送系统中压缩封闭系统的流体填充的人工晶状体(如图1A-1C和图2所示)时,人工晶状体最靠近插入器主体4的远端5的部分会比人工晶状体的其余部分先开始受压缩。当人工晶状体的远端开始受压缩时,容纳在人工晶状体内的流体通常被挤压或者被迫使进入人工晶状体的更靠近侧定位的部分中。此外,人工晶状体的首先从递送装置展开的部分开始扩展,同时人工晶状体的更近侧部分继续受到压缩,一些流体开始被向远侧挤压入人工晶状体的当前自由和正扩展的远侧部分中。
因此有利的是在压缩之前定向插入器主体内的人工晶状体,使得流体以预定的方式在整个人工晶状体内分配,以使人工晶状体受压缩且受到的损坏最小化。例如,图3更详细地显示了插入器主体的远端5。人工晶状体置于插入器主体中,使得前(或远侧)接触部12首先开始从插入器主体被展开。当前接触部开始从插入器主体被释放时,前接触部接收从光学部分和/或后接触部14挤压来的流体。
该实施方式可以要求带上的强的张力,因此优选的是牵引机构采用被设计用来增强机械利益的特征。杠杆、螺杆和/或棘爪可用来为使用者提供控制以及所需的力。
插入器主体总体上为带有大致渐缩形状的刚性结构,其宽度朝着远端逐渐减小,以在人工晶状体沿着远侧方向移动时压缩人工晶状体。在一些实施方式中,插入器主体的远端小于近端宽度的大约50%。这并不意味着限制,其可以小于近侧部分宽度的大约40%、大约30%、大约20%、大约10%或更少。尽管所示的实施方式仅仅包括底面,但是插入器主体还可具有顶面(带有与底面中类似的空间以避免滑动)。如果插入器主体不具有顶面,则也可在装置中包括第四带。
牵引滑块和带可由相对刚性的材料诸如迈拉(Mylar,聚酯类高分子物的商品名)制成,或由弹性体材料诸如硅树脂制成。
尽管在该实施方式中显示了三个带,但是在递送装置中可有更多带(诸如四个)或者更少带。
图4示出了递送装置的第二种实施方式。在该实施方式中,递送装置包括翻转管30,该翻转管30包括沿着管长度的至少一部分的至少一个狭槽或切槽32。此处所使用的术语“翻转”通常指至少一段管能够卷回或者折回到管上,就像一双袜子或者一条裤子的裤边那样。但是,在一些实施方式中,翻转管不具有狭槽。
此处使用的翻转既可指当管的内表面向外卷回变成管的外表面的步骤,也可以指当管的外表面向内卷回变成管的内表面。
图4显示了处于未翻转状态(没有一段管翻转或卷回)的翻转管30。显示的狭槽32与管30的纵向轴线LA平行地延伸。

图6显示了示例性翻转管30在管的包括狭槽的部分已开始翻转时的透视图。当狭槽的远端到达管的远端并且当管的周向围绕狭槽的部分开始并持续翻转时,管中的狭槽导致管的周向围绕狭槽的部分“开花”。图7显示了狭槽继续开花。图8是开花的狭槽的远端视图。一旦管的狭槽部分完全翻转,管的其余部分继续以同样的方式翻转,对管的设置在狭槽近侧的部分同样如此。正是以这种方式管中的狭槽允许管翻转时管的更大程度的扩张或张开。
在图9所示的翻转管设计的一种实施方式中,翻转管被连接到注射器式装置40。装置40包括外部主体42,该外部主体42包括内孔或通道,内部主体44穿过该内孔或通道。内部主体44包括位于其近端的手柄46。翻转管30的近端50连接到内部主体44的远侧部分45,并且翻转管30的远端52连接到外部主体42。当沿着远侧方向(例如通过向远侧推动手柄46)致动内部主体44时,内部主体44相对于外部主体42向远侧运动。由于翻转管的近端连接到内部管的远侧部分45,该运动也会使翻转管的近侧部分沿着远侧方向运动。翻转管的远端52保持连接到外部主体42并因此不运动。类似地,当诸如通过沿着近侧方向拉动手柄(或者致动内部主体44)使内部主体向近侧运动或被拉动时,内部主体44相对于外部主体42向近侧运动并因此使翻转管的近端向近侧运动。注意到,正是内部主体和外部主体的相对运动控制翻转管的运动(和由此的翻转),类似地,外部主体可在内部主体上沿着远侧方向推进或沿着近侧方向缩回,以造成相对运动。
此外,内部主体和外部主体可设置在外部护套中,使得递送装置的使用者无法看到内部主体和外部主体。内部主体和外部主体还可被连接到诸如控制钮的致动器,使用者可使用该致动器来精细地控制内部主体相对于外部主体的推进或者外部主体相对于内部主体的缩回。这可为使用者提供对人工晶状体递送的精确控制。
为了将人工晶状体递送到眼睛中,人工晶状体首先如下地被装载到如图9所示的递送装置的远端中。如图9所示,手柄46被向远侧推进(或者旋转钮,或者其它致动器,以控制内部主体和外部主体的相对运动),使得一部分翻转管被设置在外部并位于外部主体42远侧。翻转管中的狭槽暴露于或位于外部主体外部并“开花”。将人工晶状体置于开花的开口中,并然后沿着近侧方向致动手柄或者沿着远侧方向推进外部主体,或者同时进行这两个动作。由于翻转管的内层沿着近侧方向运动,导致更多的管外层向内卷并成为管内层的部分,狭槽被缩回到管外层内。由此迫使狭槽闭合并且借助管的闭合或完整部分上的环箍力压缩在管内的装置。
由于管向内翻转并且与人工晶状体一起移动(与图2和3中所示实施方式中的带类似),人工晶状体上的剪切力和张力的大小被最小化。基本上所有的滑动(以及伴生的剪切力)都发生在翻转管的两个层之间,因此在翻转管和人工晶状体之间没有滑动(或非常少)。在一些实施方式中,润滑剂被施加到翻转管,以使剪切力和张力最小化。
随着继续沿着近侧方向拉动手柄,人工晶状体随着进一步向近侧移动进入通道中晶状体继续被装载入外部主体内。在该实施方式中,随着人工晶状体被拉入翻转管中环箍力迫使人工晶状体被压缩,压缩完成。
图10显示了通过如上所述装载过程将一部分人工晶状体装载到递送装置中(并位于翻转管内)的示例性人工晶状体100的截面图。示例性人工晶状体100是柔软、挠性的可调节型人工晶状体,该可调节型人工晶状体包括光学部分102、以及具有与光学部分流体连通的接触部12和14的周边部分。人工晶状体包括在接触部和光学部分之间传递以响应于睫状肌的运动调节人工晶状体的流体。
当被压缩成递送构型时,人工晶状体100的长度增加(如图10所示)而同时变窄。当被压缩时,人工晶状体内的流体被从首先装载的人工晶状体部分挤压。如图10所示,近侧或后接触部14首先被装载,这将流体从近侧接触部挤压入光学部分(并同样地还进入远侧接触部12)。随着光学部分被装载入递送装置(例如,随着手柄被继续向近侧拉动),光学部分受到翻转管的压缩,光学部分中的流体被挤压入远侧接触部12中。
图10显示了人工晶状体处于装载或递送构型。远侧接触部12位于递送装置外部并且容纳了比人工晶状体处于扩展构型时其所容纳的更大体积的流体。类似地,光学部分102和后接触部14容纳的流体比它们处于扩展构型时容纳的流体少。在该递送构型,人工晶状体已部分压缩并变细长,很多流体已被挤压入远侧或前接触部。
为了将人工晶状体展开到眼睛中(例如在天然晶状体已被移除时展开入晶状体囊中),推动远侧或前接触部通过角膜切口并进入囊中。之后内部主体42被向远侧推动(或内部主体被向近侧拉动,或者二者都进行),这导致翻转管和被装载的人工晶状体一起向远侧移动,从而通过挤出翻转管的开花狭槽部分而将人工晶状体从递送装置展开入眼睛中。随着人工晶状体的光学部分开始从外部主体释放,流体从远侧接触部运动到光学部分,导致光学部分的体积膨胀。之后,随着近侧接触部从递送装置释放,近侧接触部开始重新填充流体并增大体积。一旦人工晶状体已被完全展开到递送装置外部(并进入囊中),人工晶状体大致恢复到其预装载、大致扩展的构型(尽管人工晶状体的形状由于晶状体囊作用在人工晶状体上的力而在植入后略微改变)。
图11A-11D显示了递送装置的替代实施方式70,该递送装置70包括外部主体72和带有钮76的内部主体74。为了装载人工晶状体旋转钮76,这样沿着近侧方向致动内部主体74和/或沿着远侧方向致动外部主体。为了展开人工晶状体旋转钮76,这样沿着远侧方向致动内部主体和/或沿着近侧方向致动外部主体72。护套73罩住外部主体72并为医生提供工作时使用的稳定手柄。图11D显示了翻转管78的远端77的放大透视图。
在图11A-11D所示的实施方式中,翻转管的远端77能够改变,使其在植入过程中不会相对于眼睛移动。管将翻转(内管变成外管,或外管变成内管),但是远端保持基本上固定在原位。这是很重要的,因为使用者不必担心远端77在该过程中接触和干扰眼睛。使用者也不必担心在展开过程中递送系统的远端相对于眼睛移动。
图12A-12C显示了如上所述将人工晶状体80装载到递送装置70。人工晶状体80包括后接触部82、光学部分84和前接触部86。人工晶状体递送入眼睛中以图12A-12C所示的步骤的反序进行。
图13A-13C显示了从递送装置70展开人工晶状体。图13A显示了从翻转管的远端延伸的前接触部。图13B显示了光学部分正露出,图13C显示了后接触部几乎完全展开。图14示出了邻近人工晶状体80的递送装置70的尺寸。
在一些实施方式中,翻转管为能够翻转的薄的、坚韧的、总体上有延展性的材料。为了对管进行翻转,优选的是管通常有些延展性并且相对于管的内径而言非常薄。也可使用具有相对不同的轴向刚度和周向刚度的复合材料。例如,管可包含沿着管的纵向轴线延伸的纤维,用以在保持周向弹性特性的同时沿着轴向加强管。替代地,翻转管可通过拉拔成形,以提供沿着其长度的额外刚度。
尽管在上述实施方式中显示和描述了在翻转管中的一个狭槽,但是递送装置可具有多于一个的狭槽,诸如二、三、四、五或更多狭槽。狭槽可以有助于在装载或展开过程中使人工晶状体上的剪切力和张力最小化的任何方位沿着管的长度围绕管设置。在一些实施方式中,翻转管没有狭槽。
可以使用各种致动机构递送该装置。例如但不受此限,可以使用安装在把手上的钮、扳机或杠杆作为注射器设计的替代方式。
图15示出了替代的递送装置60,该递送装置60包括主体62、插入器64以及连接到插入器64的推进机构或致动器66。插入器64为如图16所示的沿着其长度卷起的片,其中插入器64的一个边缘与另一个边缘叠置。插入器64的近端(未显示)连接到推进机构66的远端(未显示)。随着推进机构66沿着近侧方向致动,插入器64被缩回到主体62中。当插入器64被缩回到主体62中时,主体62通常压缩插入器64。这导致插入器64的直径减小,并且片形成更紧的卷或者蜷曲。
图16是插入器的远端视图,并且图17显示了其中插入器64缩回到主体62中的主体62的远端透视图。
为了将人工晶状体装载到递送装置60中,推进机构被向远侧推动以从主体62的远端展开插入器64(如图15所示)。插入器主体的远端将呈更加开放(即蜷曲不那么紧)或者第一构型,从而允许人工晶状体被置于插入器的远端中。在将人工晶状体置于插入器的远侧开口中之后,向近侧拉动推进机构(或者向远侧推动主体62)。这将插入器拉入主体62中,由此主体62将压缩力施加在插入器上,导致插入器更加紧密地合拢到自身上。由此插入器将压缩力施加到人工晶状体上。如图上述其它实施方式一样,由于人工晶状体与插入器一起向近侧移动,人工晶状体在插入器内压缩。插入器和人工晶状体一起移动,因此使作用在人工晶状体上的剪切力和张力最小化。
一旦装载入递送装置中,人工晶状体就可如上所述地通过伤口插入。
一旦主体62已推进到伤口中,推进机构66向远侧推进,这开始从主体展开合拢的插入器。随着插入器被推出主体62,人工晶状体与插入器一起移动。随着插入器被从主体62推动,其开始展开或打开,从而允许光学部分和后接触部开始扩展并且再次填充当人工晶状体处于装载递送构型时被挤入前接触部中的流体。
该实施方式可与附加的第二推进机构一起使用,以从卷绕的插入器进一步推进人工晶状体。例如,柱塞状装置可被设置在推进机构的内孔或通道中。柱塞状装置可被向远侧推动通过推进机构,以与人工晶状体接触,从而从合拢的插入器完全展开人工晶状体。由于在插入器被向远侧推动得尽可能远之后人工晶状体处于大致未受压缩状态,仅仅需要很小的附加力就可从合拢的插入器完全推动人工晶状体。因此柱塞状装置不会损坏人工晶状体。
替代的第二推进机构使用液压力来从合拢的插入器完全展开人工晶状体。推进机构内的腔可被用于在插入器内传递流体,由此迫使人工晶状体离开插入器。流体也将使作用在人工晶状体上的剪切力或张力的大小最小化。诸如插塞或其它插入物(诸如硅树脂材料)的密封机构也可被设置在卷绕的插入器内,以帮助在人工晶状体和插入器之间形成密封,以辅助人工晶状体的液压弹出。
通常,卷绕的插入器是非常薄的材料。在一种实施方式中,卷绕的插入器包括迈拉并且大约0.004英寸厚。插入器的截面可呈多种截面形状,诸如圆形、卵形或椭圆形。
图18A-18E示出了用于装载和递送人工晶状体210的装载和递送系统300的实施方式。该系统包括刚性的外管302、柔性的内管304(如图所示分裂成两个半部)和压缩器夹306。外管302能够穿过眼睛中的大约4mm的切口装配。外管302连接到外管致动器322,内套管304连接到内套管致动器324。外管和内套管可通过致动外管致动器322和内套管致动器324中的一者或二者来相对于彼此轴向运动。压缩器夹可略微地粘接(例如使用诸如Loctite495的弱粘接材料)或者不粘接到内套管。
为了将晶状体310装载到外管302,人工晶状体首先如图18A所示地(也如图18B更详细显示地)定位于系统中。接触部312首先相对于光学部分314轴向定位(一个接触部在前,另一个接触部在后)。这有助于装载过程。之后沿着大致箭头C方向的压缩力被施加到一个或两个压缩器夹306上。可通过钳子或能够将两个元件聚到一起以使压缩力C施加到压缩器夹的其它类似装置来施加压缩力。结果,压缩力被施加到晶状体,并导致晶状体在内套管的两个半部之间受到压缩。内套管而不是压缩器夹与晶状体接合。施加压缩力,直到内套管的两个半部聚到一起,使得晶状体在内套管的两个半部内完全压缩。压缩器夹受到压缩,直到它们彼此接合或者压缩器夹的边缘之间略有间隙。在压缩过程中,晶状体受到压缩并变细长。
在压缩器夹被压缩到如图18C所示的闭合(或基本闭合)位置后,外管致动器322沿着箭头D的方向向远侧推进(如图18D所示),而内套管致动器324保持在原位。外管致动器322的运动导致外管在内套管(保持在原位)上向远侧推进。也可将内套管向近侧缩回,而将外管保持在原位。推进外管使压缩器夹沿着远侧方向移位,压缩器夹还相对于内套管运动。推进外管,直到内套管(以及由此晶状体)被置于外管内,如图18E所示。在该装载步骤中,在外管和内套管之间发生滑动,而在晶状体和内套管之间没有滑动。这使得作用于晶状体上的剪切力和张力最小化。
之后外管被推进通过眼睛中形成的切口。为了将晶状体从递送系统展开到晶状体囊中,内套管致动器324被沿着箭头D方向向远侧推进。这导致内套管相对于外管向远侧推进。随着内套管从外管的远端露出,内套管开始沿着狭槽分裂,晶状体开始扩展。由此晶状体可被递送到囊中。
外管是大致刚性的,并且在一种实施方式中是不锈钢管。内套管是大致挠性材料,并且在一种实施方式中是PTFE(聚四氟乙烯)。压缩器夹可以是任何合适的刚性材料。
增大外管体积可增大晶状体被压缩到的体积。通常希望外管具有尽可能最大的横截面积,同时仍然允许外管被推进到尽可能最小的切口中。已经发现,使用大致椭圆形横截面的外管能够提供穿过最小切口的最大横截面积。
在替代实施方式中,如图18A-18E所示的内套管可由如图15-17所示的诸如插入器64的卷绕片替换。系统以与上述参照图18A-18E描述的方式类似地工作。
图19A-21显示了液压晶状体装载系统的替代实施方式。使用液压系统将人工晶状体装载到递送装置(以及使用液压系统展开人工晶状体)能使作用于晶状体上的剪切力和张力最小化。使用一般润滑液或流体迫使晶状体进入递送装置,这在晶状体受到压缩并变细长时使作用于晶状体上的剪切力和张力最小化。图19A显示了用于将人工晶状体402装载到装载管408中的装载系统400。该系统包括注射器404,注射器404包括柱塞406。注射器404的远侧区域412包括渐缩的内表面410,其在远端具有小于近端的横截面直径。注射器的远侧区域容纳晶状体以及流体414。流体可以是诸如生理盐水的液体并且包括或可以是已知的粘弹性润滑剂,诸如例如但不限于透明质酸钠、羟丙基甲基纤维素、硫酸软骨素的水溶液。
为了将晶状体推进入装载管408中,沿着远侧D方向致动柱塞,这导致流体414和晶状体402朝着装载管108向远侧推进。柱塞继续被向远侧推进,直到迫使晶状体穿过装载管108的近端416。通过使用润滑材料移动晶状体,作用于晶状体上的剪切力和张力被最小化。
图20B显示了用于将人工晶状体602装载入装载管608的替代的液压装载系统600。该系统与前面的实施方式类似并包括带有柱塞606的注射器604。注射器包括晶状体室612,该晶状体室612具有大致圆形形状,以保持大致圆形的晶状体602。注射器还包括渐缩部分610,该渐缩部分610将晶状体引导到装载管608。晶状体602初始被定位在晶状体室612中,并且远侧接触部605从光学部分603向远侧延伸并进入渐缩部分610。当流体614被迫使穿过晶状体室612时,这种初始定位帮助将晶状体引导成在装载管608内的压缩构型。柱塞被向远侧推进,以将流体引导穿过晶状体室612,这迫使晶状体602进入装载管608。
在替代的设计中,人工晶状体可在真空压力下被装载入装载管中。
在晶状体被装载入装载管中之后,晶状体被以液压方式递送到眼睛中。装载管首先从装载设备拆下。然后装载管被插入穿过眼睛中的切口,流体(诸如润滑流体)通过装载管被引导,以将晶状体从装载管弹出到眼睛中。液压展开也使得作用于晶状体上的剪切力和张力最小化。可使用注射器引导流体通过装载管。替代地,小活塞沿着管驱动,从而推动位于活塞远侧的一小段流体。使用诸如钮、杠杆、棘爪等的致动器控制活塞。活塞可连接到装载管的任一端。这意味着晶状体可从其被装载的同一端弹出或者可从装载管的另一端展开。
图21示出了使用蠕动运动装载人工晶状体(未显示)的替代装载系统设计。在该设计中,晶状体上的纯粹的压缩负载及时地与晶状体上的剪切负载分离。晶状体“慢慢运动”成完全压缩状态。系统700包括刚性的大管702、刚性的小管706和具有大致锥形或渐缩形状的挠性管704。流体710容纳在系统内以润滑该系统并还帮助推动晶状体穿过系统。晶状体从刚性大管702移动通过挠性管704并在刚性小管706内进入完全压缩状态。大管702具有比小管706大的直径。在压强P1和P2之间(P1较高)通常存在压强梯度。在压强P1和P2之间的压强差(为驱动压强)等于P1减去P2。来自挠性管上的压缩力的压强P3用于沿着基本垂直于轴线A的方向压缩晶状体。P3的脉动与同样为脉动式的驱动压强异相。为了装载晶状体,P3初始地被增加以径向压缩晶状体。之后P3在驱动压强增加时减小,这样装置被沿着方向D推动一小段距离并再次径向扩展。当P3减小时,挠性壁沿径向远离晶状体移动,剪切力被减小。然后P3再次增加,径向压缩晶状体。之后在驱动压强增加时P3减小,这再次使晶状体沿着D方向移动。由此,晶状体沿着远侧方向D以小增量移动,在移动时受到压缩。反复进行这种移动,直到晶状体在小管706内完全压缩。然后可使用此处描述的任何方法展开晶状体。
在此处描述的任何或所有实施方式中,递送方法包括在眼睛中形成伤口,伤口通常包括在眼睛中的切口。在一些实施方式中,切口为大约4mm,优选为大约3.7mm。但是,切口可以略大或略小。
在此处描述的任何实施方式中,人工晶状体的位置和/或方向需要针对装载步骤进行调节。例如,当装载带有接触部的人工晶状体时,可能需要对准接触部,以使得它们在晶状体受压前大致沿着递送装置的纵向轴线定向(参见例如图18B)。替代地,仅仅需要使一个接触部调直,而第二个接触部可围绕光学部分周边定位(参见例如图21)。这些方向可提供更好的递送轮廓并且使展开过程中对人工晶状体的损坏几率最小化。
为了压缩此处描述的任何流体填充的可调节型人工晶状体,需要施加大约0.5英镑的侧向压缩力。但是,这取决于人工晶状体的尺寸、构成和体积是可变的。
尽管仅仅说明了这些实施方式,但是它们都旨在使装载和/或递送过程中作用于人工晶状体上的剪切力和张力的大小最小化。一种共用的方法是使人工晶状体和递送系统组成元件之间发生的滑动的量最小化。在本发明中包括了其它实施方式,其允许使用(或结合)递送装置组成元件将人工晶状体装载到递送装置中和从递送装置展开,以减小那些不期望的力。

Claims (12)

1.一种装载人工晶状体的方法,该方法包括:
提供人工晶状体,其中具有可流动的介质,该人工晶状体邻近递送装置在压缩室内,其中所述压缩室和递送装置流体连通,所述人工晶状体包括光学部分、远侧接触部和近侧接触部;
定位所述远侧接触部,使得远侧接触部从所述光学部分向远侧延伸,同时所述近侧接触部不从所述光学部分向近侧延伸;以及
将人工晶状体装载到递送装置中,其中装载包括重新分配所述人工晶状体内的可流动的介质。
2.如权利要求1所述的方法,其中,将人工晶状体装载到递送装置中包括将人工晶状体的光学部分从未受压的扩展构型压缩到受压的递送构型。
3.如权利要求2所述的方法,其中,压缩光学部分包括增加人工晶状体的长度。
4.如权利要求1所述的方法,其中,所述可流动的介质为流体,并且其中装载步骤包括重新分配人工晶状体内的流体通道或流体室内的流体。
5.如权利要求1所述的方法,其中,将人工晶状体装载到递送装置中包括将可流动的介质从人工晶状体的远侧接触部内重新分配到光学部分内。
6.如权利要求1所述的方法,其中,定位所述远侧接触部包括将人工晶状体定位在压缩室内,使得人工晶状体的光学部分以大致未受压构型处于压缩室内。
7.如权利要求1所述的方法,其中,所述压缩室的宽度大于人工晶状体的处于未受压扩展构型的光学部分的直径。
8.如权利要求1所述的方法,其中,可流动的介质通道或可流动的介质室位于人工晶状体的光学部分内。
9.一种将人工晶状体装载到递送系统中的方法,该方法包括:
提供人工晶状体,人工晶状体包括与远侧接触部和近侧接触部流体连通的光学部分,并且可流动的介质能够在远侧接触部、近侧接触部和光学部分之间运动;
将人工晶状体定位在压缩室内,使得远侧接触部从所述光学部分向远侧延伸,同时所述近侧接触部不从所述光学部分向近侧延伸;以及
将人工晶状体从压缩室装载到递送装置中,其中装载人工晶状体包括将可流动的介质从所述光学部分内重新分配到所述远侧接触部内。
10.如权利要求9所述的方法,其中,装载人工晶状体包括使得流体流入压缩室。
11.如权利要求10所述的方法,其中,装载人工晶状体包括使得流体从压缩室流入递送装置。
12.如权利要求9所述的方法,其中,定位步骤包括将光学部分以大致未变形构型定位在压缩室内。
