2142104 9406357 PCTABS00030 A device (10) for retrieving foreign bodies (26), such as vasoocclusive coil or elements, from within vessels (25) such as those of the cardiovascular system comprising a tubular member (27) and a flexible wire (19) that extends axially through the tubular member with its distal end (22) extending out the distal end of the tubular member and looped back and affixed to the distal end (18) of the tubular member to form a loop (24) whose size may be adjusted by axial movement of the wire relative to the tubular member and which may be used to ensnare the foreign bodies and remove them from the vessel.

This invention relate~ to a device for capturing a~d removing bodie~ or articles from within a vessel, such as tho~e of the cardio~a~cular system or the genitourinary tract.

Various loops and baqket configura~ions have been u~ed to remove calculi ~rom the biliary or urinary system. See, for insta~ce, U.S. Pat~nt No. 5,064,428.
Embolization o~ ves~els i~ becoming of increasing therapeutic importance in treating conditions such a~ arterio~ascular malfoxmatio~s, a~eurysms, fistulas, Yascular tumors, and the like. The procedure in~olves placing foreig~ bodies such a m~tal coils, balloons, bead~, and the like i~to the ~e~sel. ~t times the~e bodie~ become errant or i~ i8 de~ired to retrieve and re~ove them from the ves~el for other rea~ons.
Existing snare~ are too ~tlff and/or ~oo large to pe~mit I deep tis3ue acces~ through~tortuous ~es~el pa~hs. iThe present in~ention provides a device for retrie~i~g bodie~

Disclo~ure of ~hQ Inven~i~n The invention i3 a medical retrieval device for capturing and remo~ing a body from withi~ a ve~el ~`
comprising:
(a) a first elongated member having a proximal ~-end, a di~al end, and a lumen extending ther~between, ~aid fir~t elongated member being c~pable of being advanced through the vessel to the site of the body; and (b~ a second elongated member having a proximal end and a flexible distal end that i8 received through the lumen with the di~tal end of the second elongated member extending distally of the lumen and being looped back and affixed to the di~tal end of the first elongated m~mber to form a loop whose siæe may be adjusted by æial movement of thP ~econd elongated member.

Figures 1 and 2 depict a preferred embod ment ~f the retxieval device of the i~ention. The retrie~al ~ ;~
device as a whole i~ generally designated 10. Its principal components are: a proximal e~d itting 11; a .

WO9~/063~ 210 ~ Pcr/~lss3/oos catheter tube 12; and a flexible wire 13 that extend3 ax.ially through the fitting and catheter tube.
Fitting 11 has a con~entional stxucture that i3 commonly u~ed at the proximal end of a catheter. It has a main body and an integral side arm 15. The main body 14 ha3 a central axial lumen through which the flexible wire 13 i9 received. The 3ide arm has a lumen which connects into the lumen of the main body. The proxi~al end of the side arm is connected to a ~ource (not chown}
of a physiologically acceptable fluid. The 30urce prefer~bly provides the fluid under pres3ure. Typically the fluid will be introduced in~o the side arm via a syringe or a tube connected to a pressurized intravenous fluid bag (not shown) connected thereto.
The proximal end 16 of catheter 12 is ~onnected to the distal end of fitting 11 with the lumen 17 of the catheter communicating with the lumen of main body 14.
The structure a~d size of catheter 12 i8 gUCh as to permit it to be i~serted within the ~mall ve3~el~ of the peripheral vascular system. Preferably the catheter is of the structure and 3ize described i~ U.S. Patent No.
4,955,~62, th~ disclosure of which is incorporated herein by reference.
Wire 13 axtends axially through the entire length of catheter and extends out of the distal end 18 (~ee Fig. 2) of the catheter. ~ne structure and dime~sion~ of wire 13 are similar to tho~e o~' a conv~ntional guidewire. makes the e~d visible radiographically. Coil 20 serve~
to protect the distal end 19 ~rom kinking when it i9 manipulated. Typically about 0.5 to 20 cm of the distal end will be wrapped with coil. The di~tal tip 22 of the wire i~ affixed to the exterior of the di~tal end of the catheter by means of a tightly wrapped metal coil 23 or metal band that ex~end~ around the circumference of the tip of the catheter. It may also be affixed to the interior wall of the catheter provided th~ lumen i8 large enough to permit same, or it may be affixed to coil 23 by solder and/or to the catheter wall by fusion with the plastic formi~g the catheter. Coil 23 is prPferably made of a rad}opaque metal ~uch as platinum so that the location of the end of the catheter i9 visible radiographically. In the event coil 23 does not ~erve a~
a radiopaque marker, a separate radiopa~ue band or coil may be affixed to the tip of the catheter. The af~ixation of the tip of wire 1 to the catheter forms the distal end of the wire into a loop 24, the size of which may be altered by axial movement (represented by the double-headed arrow in Fig~. 1 an~ 2) of wire 13.
The extended confi~uration of loop 24 is shown in solid and dashed lines in Fig. 2 and the retracted configuration of loop 24 i9 shown in phantom li~es in Figure 2.
Figures 1 and 3A-D show how the retrieval device i9 u~ed to retrieve a body from within a vessel.
As ~een in Figure~ 1 and 3A, the de~ice i~ inserted into a ves~el 25 ~o that the distal end of the de~ice is 1 ad~acent to the hody to be remo~ed from the ~esseli in the depicted case a ~a~oocclu3ive coil 26. The inser~ion of the device to ~uch a location i~ acco~pli~hed in a conventional man~er. catheter 27 may be placed in the vesciel using a ~eparate guidewire (not shown~ and the catheter-wire assembly of the invention inserted ~o the ~ite ~ia catheter 27. Once the device i9 80 positioned loop 24 of the device is enlarged by distal advancement of wire 13 while holding catheter 12 stationary. In order to facilitate the axial manipulation of wire 13 within tube 12, a physiologically acceptable fluid ~re~resented by arrows 28 i~ Figureq 3A-3D) may be pumped through the lumen of catheter 12 from side arm 15 -- thus providing lubrication between the wire and the catheter. ~oop 24 i~ manipulated within the ~essel to en~nare (see Fig. 3B) coil 26. Modifications of the above-described modes for carrying.out the invention that are ob~ious to those of skill in the medical device art are intended to be within the scope of the followi~g claims.

Claims (12)

Claims
1. A medical retrieval device for capturing and removing a body from within a vessel comprising:
(a) a first elongated member having a proximal end, a distal end, and a lumen extending therebetween, said first elongated member being capable of being advanced through the vessel to the site of the body; and (b) a second elongated member having a proximal end and a flexible distal end that is received through the lumen with the distal end of the second elongated member extending distally of the distal end of the first member and being looped back and affixed to the distal end of the first elongated member to form a loop whose size may be adjusted by longitudinal movement of the second elongated member relative to the first member, said distal end of the second elongated member being wrapped with coil.
2. The device of claim 1 including (c) a radiopaque marker member carried on the distal end of the first elongated member.
3. The device of claim 2 wherein the radiopaque marker member is made of metal.
4. The medical retrieval device of claim 2 wherein the distal end of the second elongated member is affixed to the exterior of the distal end of the first elongated member.
5. The medical retrieval device of claim 1 including means for passing a physiologically acceptable fluid in a distal direction through said lumen to provide lubrication between the first elongated member and second elongated member.
6. The medical retrieval device of claim 5 wherein said means includes (i) a proximal end fitting to which the proximal end of the first elongated member is attached said and fitting having an axial lumen that communicates with the lumen of the first elongated member and through which the second elongated member extends and (ii) a source of said physiologically acceptable fluid.
7. A medical retrieval device for capturing and removing a body from within a vessel comprising:
(a) a catheter body having a proximal end, a distal end, and a lumen extending therebetween, said catheter body being capable of being advanced through the vessel to the site of the body; and (b) an elongated member having a proximal end and a flexible distal end that is received through the lumen with the distal end of the elongated member extending distally of the catheter lumen and being looped back and directly affixed to the distal end of the catheter body to form a loop whose size may be adjusted by longitudinal movement of the elongated member relative to the catheter body.
8. The device of claim 7 further including (c) a radiopaque marker member carried on the distal end of the catheter body.
9. The device of claim 8 where the radiopaque marker member is made of metal.
10. The device of claim 7 where the distal end of the elongated member is affixed to the exterior of the catheter distal end.
11. The device of claim 7 including means for passing a physiologically acceptable fluid in a distal direction through the catheter body lumen to provide lubrication between the catheter body and the elongated member.
12. The device of claim 11 where said means includes a proximal end fitting to which the proximal end of the catheter body is attached, said end fitting having an axial lumen that communicates with the catheter body lumen and through which the elongated member extends and said physiologically acceptable fluid flows.
